Dear Mr. Starostecki:

Your letter of February 18, forwarded combined Inspection Reports 50-277/82-01 ar d 50-278/82-01, Appendix A addresses an apparent violation of an NRC requirement and is categorized as Severity Level III in accordance with Federal Register Notice 45 FR 66754 (October 7, 1980).

The apparent violation is restated below with our response.

A.1 10CF771.5 prohibits delivery of licensed materials to a carrier for transport unless the licensee complie.= with 49CF R170-189.

49CFR173. 24 (a) requires that a package used in such transfer have its contents limited so that, under normal transport conditions, the packaging effectiveness will not be substantially reduced.

Contrary to the above, on January 5, 1982, a package containing 0.44 mil 11 curies of licensed radioactive material was delivered to a carrier for transport and its contents were not sufficiently limited:

Starostecki Page 2 substantially reduced during normal transport when a puncture of about 2-inch diameter was made in the container's side by material within the package.

Fusponse On January 5, 1982, eight boxes specifically designed to ship the General Electric clad removal equipment were packaged at Peach Bottom for shipment to Carolina Power and Light Company's Brunswick Station.

While in transit, the equipment apparently shif ted in one of the boxes resulting in a small (2 inch) puncture in the side of the box.

When the driver noticed the damaged box, he stopped and contacted the PBAPS site per his instructions.

Since the truck was within a relatively short distance from the Brunswick site, the driver was instructed to cover the hole and the piece of protruding equipment with tape and proceed carefully on to Brunswick.

Philadelphia Electric Company contacted the Brunswick Station and notified them of the occurrence.

Upon arrival at Brunswick Sta tion, surveys were taken of the box and truck and no contamination of either was detected.

Philadelphia Electric directed General Electric, the subcontractor who had performed the supervision and packing of their equipment at the Peach Bottom Station, to determine the cause of the apparent failure of the shipping container and to provide the corrective actions necessary.

4 General Electr.

Company's reply stated that GE personnel witnessed the opening of the shipping container and determined that the box had been improperly packed.

Specifically, certain blocking had not been installed allowing the machine to shift within 2he box and causing the lead screw to protrude through the side of the container.

GE has held detailed discussions regarding this incident with personnel and management within I&SE and the Apparatus Service Business Division to assure that similar incidents will not occur in the future.

GE has committed to:

1.

Alerting each I&SE District Office involved in clad removal work to the incident and taking steps (procedures or instructions) to' prevent recurrence.

2.

Alerting each Nuclear Service Shop of the details of the incident (exactly which blocking was omitted) and taking

Starostecki Page 3 additional measures (instructions, checklists, etc.) to prevent recurrence.

Peach Bottom personnel involved with shipping contaminated materials have been appraised of the incident and cautioned to be watchful over our subcontractor's and our packaging work in the future.
