This refers to the routine safety inspection conducted by Ms. E. R. Matson of this office on September 1, 1981, of activities authorized by NRC Byproduct Material Licenses No. 34-01954-01 and No. 34-01954-02 and to the discussion of our findings with you and members of your staff at the con-clusion of the inspection.

The inspection was an examination of activities conducted unde your license as they relate to ;adiation safety and to compliance with the Commission's rules and regulations and with the condit. ns of your license. The inspec-tien consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

During this inspection, certain of your activities conducted under License No. 34-01954-01 appeared to be in noncompliance with NRC requirements, as specified in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required. No items of noncompliance were identified against License No. 34-01954-02.

This inspection also included a measurement of the output of your teletherapy equipment. Our measurement was made using a Victoreen Model 570 Condenser R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80 centimeters and the field ~ size used was 10 by 10 centimeters. After applying standard correction factors for temperature, pressure, chamber factor, and timer error, our measurement of the output of your teletherapy r

equipment was 83.12 roentgens per minute. The most recent full calibration T50 s

Applying a correction factor to account for 24 days decay, the value calculated from the August 7, 1981, calibration becomes 83.67 roentgens per minute.
