Dear Mr. Seidle:
River Bend Station - Unit 1 Docket No. 50-458 Attached are results of Gulf States Utilities' (GSU) investigation (Attachment 1) into the concerns expressed by the Union of Concerned Scientists in their letter dated July 21, 1980. As a result of this investigation, all concerns stated were found to be either without merit (previously identified and currently under review) or refuted (not found to be a discrepancy).
It is also concluded that no breakdown exists in either the GSU or the Stone & Webster (S&W) Quality Assurance (QA) Pro-gram.

Allegation:
1.
" Factory-reworked areas of 600 volt power and control cables have not been qualified for nuclear application per the relevant sections of IEEE 323 (IEEE Standard for Qualifying Class IE Equipment for Nuclear Power Generating Stations) and 383 (IEEE Standard for Type Test of Class IE Electric Cables, Field Splices, and Connections for Nuclear Power Generating Stations),
and will not (be) before the cable is shipped to the jobsite (Aug. 25). Once on site, the pressure to install this cable, despite (its) lack of full pedigree, will be immense. And if defective cable, or cable with defective factory rework, is installed, and if at some future time this cable fails duri:;g a serious plant condition, we could be in the soup all over again - another TMI!" The vendor of this cable is Okonite Company and it " routinely makes repairs and splices on cable

Response
Control and 600V power cable are being procured from the Okonite Company under specification nos. 241.240 and 241.234, respectively.
Both specifications impose the requirements of IEEE 323 (1974) and IEEE 383 (1974).
Okonite is required to submit their cable qualification procedures for review and approval. S&W Engineering visited Okonite's Brunswick facility on Feb. 21, 1980, to observe Okonite's repair and splicing operations and to review their inprocess i
procedures (Reference 1). Okonite has connitted to supplying cables, fully meeting the specification requirements, and has undertaken qualification work pertaining to factory repairs / rework and spliced cable with test results being available in early 1981 (Reference 2).
Existing procedures and controls provide for the fabrication, shipping, and installation of cable prior to the completion of all qualification testing.
For any cable not meeting the qualification test, appropriate action will be taken.
As stated in the River Bend PSAR, the schedule for this engineering effort to support IEEE 323 and IEEE 383 equipment qualification will generally follow equipment purchasing schedules beginning Post CP and continuing through the aperating licensing stage.
It should also be noted that a fabrication hold has been in effect at Okonite since 6-26-80 (Reference 2A),
due to a S&W QA audit, which identified inspection deficiencies.

"No position has yet been written on Reg. Guide 1.131."
NRC Regulatory Guide 1.131, Qualification Tests of Electric Cables and Field Splices for Light-Water-Cooled Nuclear Power Plants, endorses with modifications IEEE 383.
In other words, GSU has not yet decided the manner in which (or extent to which) it will comply with requirements for qualifying electric cables and splices, despite the fact that construction has been under-way for three years.
Response
Presently all Category I (safety related) specifications for elec-tric cable require compliance to IEEE 383-74. These specifications The River Bend PSAR currently are reviewed and ap(proved by GSU.1974) for electrical cable qualification Regu-endorses IEEE 383 latory Guide 1.131 was issued by NRC for review and comment only.
Ski Corporation has reviewed Regulatory Guide 1.131 and has issued an S&W Engineering Corporation Regulatory Guide Position (Decenber 19, 1979) (Reference 3).
Also, a S&W River Bend Project review was initiated March 24,1980 (Reference 4), to develop a Regulatory Guide 1.131 position which is currently under review.

"The conductor tests on thermocouple extension wire and on 300 volt instrument cable will be performed on a bulk basis and there will be no direct traceability from the conductor to the power plant."
Response
Specification no. 241.234 for thennoccuple extension wire requires.
an EMF conductor test to meet ANSI C96.1. Rockbestos performs the required EMF test on each reel of wire to meet this requirement.
Rockbestos has connitted to maintain traceable receiving inspection reports which verify that EMF values are within standard limits of error per ANSI C96.1. Test reports submitted with each shipment shall be traceable to incoming receiving inspection reports and to each reel tested (Reference 5).
Specification nc. 241.242 for instrument cable requires onductor test to meet IPCEA S-19-81, section 6.3; ASTM 88; B33; and 8189.
Rockbestos certifies that their conductor meets the specification requirements except that conductor resistance testing on all completed cable is performed in lieu of resistivity testing of copper strands (Reference 6).
This is acceptable to S&W and the specification is being revised (Reference 6A).
There is 100 percent traceability from the conductor to the reel through the resistnace test reports for each reel.
The acceptability of a Certificate of Conformance in lieu of test reports currently required in the specification is still under evaluation.

The vendor, Husky products, has provided repair instructions, but none of the repairs proposed have met the seismic quali-fication criteria."
Response
Approximately 17,000 pieces of cable tray have been delivered to the site as of July 28, 1980.
Eighty-two p'ieces of the cable trays received at the site have been rejected due to damage.
Of the 82 pieces, 37 have been dispositioned as either scrap or return to vendor on Unsat Field Quality Control Receiving Inspection Reports (Reference 7 through 11)., The disposition of the remaining 45 pieces is on hold pending the approval of N&D 9753 (Reference 12).
N&D 9753 identifies the acceptable cable tray repairs but is awaiting approval pending review of seismic acceptability of repairs submitted by Husky (Reference 13).

"Large quantities of cable tray have been shipped without Husky ever having complied with the specification require-ment that their seismic qualification criteria must be ap-proved by a professional engineer."
Response
Specification no. 241.320 contains the documentation and quali-fication requirements for Category I (safety related) cable trays supplied by Husky. The specification in effect at the commence-ment of shipment of cable tray was dated March 1,1974, including Addendum no. 5 (May 2,1979) (Reference 14). Addendun no. 5 deleted the requirement for a signed seismic qualification certificate prior to shipment.
Cable trays were shipped starting in December, 1979.
S&W Engineering approved Husky's seismic calculations and load test on November 12, 1979 (Reference 15). Clarification concerning the seismic qualification certificate as a part of the shipping documentation package was obtained by S&W QA from S&W Engineering on April 17, 1980 (Reference 16). The seismic certification approved by a Professional Engineer was completed by Husky on May 20, 1980, and transmitted to S&W Engineering on June 5, 1980 (Reference 17).

"The power run to the makeup water structure is done with two entirely different type cables, thereby violating good engineer-ing practice and possible compromising the ground system."
Response
The power run to the makeup water structure is Category II (non-safety related) (Reference 18).
The cables are not entirely dif-ferent types.
Both designs are basically the same in terms of rating, size, and insulation material. One is a single conductor type for ductbank and cable tray application.
The cable specified is three 1/C-250 MCM Cu, EPR insulation with insulation shielding, 15-kv class.
The other is a three conductor type with overall Jacket for direct burial application.
The cable specified is 3/C-250 MCM Cu, EPR insulation with insulation shielding on individual cnnductor, overall jacket,15-kv class.
No compromises exist in the grounding system (Reference 19).
Each single conductor shield is to be grounded at one end at the point of transition to the three conductor cable shield, and the three conductor cable shield is to be grounded at both ends.

Cable tray with a rung removed has not been proven to be seismically qualified."
Response
Specification no. 248.000 contains the requirements for cable tray installation. The specification allows the removal of a cable tray rung to facilitate cable drops subject to approval by the Engineers (Reference 20).
This approval would be documented on an Engineering and Design Coordination Report and contain the technical justification for such removal.
S&W is currently reviewing the seismic acceptability of removing a rung submitted by Husky on June 23, 1980 (Reference 21).
Noscable trays have been installed to date.

" Power cable runs have been sized and specified on the basis of calculations E112, E119, and E120, none of which has yet been approved."
Response
'The calculations referenced are for the following applications:
E112 - Sizing of 15-kV direct burial cable - Category II (non-safety related)
E119 - Sizing of ground leads - Category II (non= safety related)
E120 - Sizing of 15-kv and 5-kv cables - Category I, II, III (safety related)
All three of the above-referen:ed calculations are in the process of being completed (Reference 22).
Preliminary calculations may be utilized as input sources to estimate cable sizes and are later verified by formally issued calculations.. Calculation nos. E112 and E119 were utilized as preliminary input to specific runs and sizes for Category II applications.
Calculation no. E120 has not been utilized for sizing or specifying power cable runs.
Cate-gory I power cable runs were sized and specified utilizing completed calculation no. E46H.
Calculation no. E120, when completed, will supersede E46H.
EAP 5.3 is the control procedure addressing control and utilization of calculations (Reference 23).

Response
Specification no. 241~.232 is the purchase specification for 15 kv cable. There is no Category I (safety related) application for 15-kv cable (Ref. 24). A review of specification no. 241.232, Rev. I against design criteria no. 241.200, Rev. I for the sizing of 15-kv cable showed that the cable sizing for 15-ky cables speci-fied in 241.232 is in full compliance with the design criteria (Ref. 25).

Workminship was good.

In both cases, procedures versfollowed to the letter; Work =anship was good.
Procedures required slightly more insulation on the patch than the rest of the cable (undoubtedly a safety margin).

Insulation and jacket materials used in patches and repairs were identical to the same material as the rest of the cable.

Overall jacket repair was on 2/c #16 ILPE-In.=ulation and Hypalon jacket.
Workmanship was good.

We expect Okonite to furnish cables which shall be free from defects in design, workmanship and material to the River Bend jobsite on schedule in the sizes and quantities specified.
We intend to hold Okonite re-sponsible for the performance of the cable, regardless of the results of the qualification testing on factory reworked esbles.
Our position is that since Okonite does routinely perform. rework on the esble, and I
there is no distinction between a virgin cable and a reworked cable ir.
I terms of quality of the product, Okonite must assume full responsiblity for qualifying the cables for River Bend Project..

v Program Overview The following implementation program to meet the qualification requirements of IEEE 323-1974 for class IE electrical equipment represents Gulf States commitment to IEEE T
323-1974 and is provided to conform to all the requirements of that document.
That is, all class IE electrical equipment for River Bend Station will be qualified to IEEE 323-1974 as
=
implemented by the following program, which is described in two 9
parts:
part A discusses implementation and scheduling; part B
discusses Gulf States plan to handle the aging requirements of IEEE 323-1974.
This program is provided in response to the NRC staff requirements as stated in the River Bend SER section 7.9.
Part A Where
- possible, Gulf States will purchase class II electrical equipment which has been aged a nd type tested in accordance with IEEE 323-1974 Our direct contact with industry
~
and equipment suppliers, however, leads us to believe that at the present
- time, valid and correlatable accelerated aging ~can only be demonstrated for a very limited number of simple. systems, i.e.,
insulation systems for wire and cable and some types of motors.
Because of this limitation in the state of the art of existing qualification technology (specifically valid application of aging to determine qualified life), it is necessary to review present technology and develop alternate qualification techniques as described in part B
of this program.
In addition to purchasing, where pos sible, equipment aged and type tested to IEEE 323-1974, Gulf States will, where possible, purchase class IE motors type testad to IEEE 311-1974, class IE valve operators type tested to IEEE 382-1972, and wire and cable type tested to IEEE 383-1974.
Gulf States is working with its NSSS vendor and Architect Engineer to develop.a ccmplete listing of class IE equipme nt for River Bend Station by type application, location, manufacturer,
- name, model
- number, part
- number, or other identifying number. From this list, as identifying numbers become available, Gulf States will establish the equipment's qualified j

-life at purchase by determining whether specific equipment has (or will have) previous operating experience, is already qualified with a satisfactory qualified life to IEEE 323-1974 by type test and/or analysis, or will require some form of on-going qualification.
The exact method of qualification for each piece of equipment can then be identified from this list.
The method 2 of qualification for such equipment can only be determined as specific manufacturers are
- known, thereby establishing known operating history, exact vendor qualification method, vendor compliarice with appropriate standards, and the necessity of -
e on-going qualification.
The schedule for this engineering effort will generally follow equipment purchasing schedules.

expect to receive NRC review and approval of this information via a continuing series of uniformly distributed meetings beginning 'i post C.P.
and continuing through the operating license stage as,
equipment vendors are defined.

As stated in IEEE 323-19.74, the preferred method of equipment qualification remains aging and type taccing.
- However, at this
- time, the industry's ability to firmly establish the qualified life of class IE equipment is limited by the ability to validly apply accelerated aging to all types of equipment.
Gulf States has therefore strived to devise alternate

methods for establishing the qualified life of.
class IE electrical equipment with the fc11owing program. To meet the requirements of section
- 6. 6 of IyEE 323-1974, the following alternate on-going qualification program is provided as allowed by section 5.5 of IEEE 323-1974.
As previously
- stated, all class IE equipment can be classified into three categories:
l 1)
Already qualified with a satisfactory qualified life to IEEE 323-1974 by type test and/or analysis; 2)
Equipment having previous operating experience; and 3)
Equipment requiring some form of on going quali-fication.
Gulf States will continue to define in detail in all its purchase specs.
the worst case envelope service ccndition anticipated for particular types of class IE electrical equipment whichever category (1, 2, 3 above) it is in.
It will be Gulf States responsibility to require that the equipment vendor compare this criteria against the qualification data or analysis already existing for his particular equipment and recommend to Gulf States a qualified operating life for that device under our particular plant service conditions.

m om w e e to cable trays which have incurred basically minor damage.$
== 1. Sharp burrs at various locations. These can generally 3 e eliminated by the application of a medium weight file. i 2. Bent rungs and side rails. Most bent rungs can be returned to their proper condition by use of a mallet and a block of wood. In those cases where the rung has been damaged beyond repair, it can be removed *from the tray and the i weld area properly deburred. This also applies to rungs ) that have been torn loose from the side rail, since it l appears that aluminum MIG welding facilities are not available for minor rework at the site. Deformed side rails can genezcally be returned to' proper shape using the same technique as with the rungs. In those instances where they have torn the material back into the splice hole it would seem advisable to cut that part away and use the
- area, tray as an odd length piece.
3. Minor cuts and gouges of the rungs or side rail that don't affect the structural integrity of the tray can, again, generally be reworked with the file to an acceptable condition. If the cable is connected to an electrical penetratien, see e ETG-IV-9, " Containment Electrical Penetrations for Shielded Power Cabic," for additional information. Short circuited operation provides a uniform approach and =4-i 3:es the i 4 possibility of operating the cable with both shield ends ungrounded. The IPCEA Ampacity Tables ETG-IV-7, for triplex an:13/C cables are based on E short circuited
- shield operation and no further derating is recuired. The
~ ampacities for the 1/C cables in these tables, however, are based on open circuited (one end only grounded) shield operation. Table A attached, lists multiplying factors which can be used to correct the 1/C cable open-circuited shield a pacities in the IPCEA tables to short circuited shield ampacities. These values are applicable to cables rated 5 ky, 8 ky, and 15 kv, with a 5 cil copper shield. 'J For those applications of power cable with shielding material, chielding thickness, or installation conditions different from Table A the following fomula may be used to calculate the ampacity =ultiplying factor for short circuited shield operation: Ampacity multiplying factor 'Rae-ocen circuited i for short circuited shield = \\ Rac-short circuited operation Where "Rac-open circuited" is the conductor a-c resistance, in ohms /1,000 ft when the shield is open circuited and "Rac-short circuited" is the conductor a-c resistance, in ohms /1,000 ft, when i.he shield is short circuited. Both a-c resistances may be obtained from the cable impedance program, see ETG-XI-18 "Cemputer Program for Cable Impedances." Where shielded cable is used, in connection with a ground sensor current transfor:er, either triplex, 3/C or a multiple of three single conductors, the cable shields should be terminated after the cable passes through the ground sensor current transfor=er with the shield connection to ground, threaded back through the transformer opening, and the ground connection made on the cable entry side of the current transfor=er. Husky S.O. 739000 - Gulf States Utilities Removal of a Cable Tray Rung Based on previous experience and testing, we have evidenced little, if any particular, difference in the failure loading of a particular style of cable tray in the vertical mode regardicss of whether the rung spacing is 6", 9" or 12" center to center. Therefore, we've used the same allowable vertical load as in the basic qualification. In the horizontal plane, we have used a factor of 15/16 x the initial transverse load to reflect the deletion of one rung at a random Iccation.
