- 1993, enclosed is the independent report " Organizational and Programmatic Assessment of the Radiological Work Practices at OCNGS" prepared by Failure Prevention and Investigation (FPI) International. Although this document.
which details the events that occurred at OCNGS on May 7 and - 11, 1993 is annotated proprietary, an agreement has been reached with FPI that this document need not be submitted under the provisions of section 2.790.
