Dear Mr. Boulette:
SUBJECT: PILGRIM INSPECTION 93-13
This refers to the safety inspection conducted by Messrs. J. Macdonald, A. Cerne, and D. Kern,
of this office on June 8,1993 to July 12,1993 at the Pilgrim Nuclear Power Station, Plymouth,
Massachusetts. Areas relevant to the health and safety of t'.e public examined during this
inspection are ocscribed in the enclosed report. Our findings were based upon observations of
performance and independent evaluations of safety systems and quality records. The preliminary
results have been discussed with Mr. L. Schmeling and other members of your staff at the
conclusion of the inspection.
Overall, safe plant operations and good attention to detail were observed by our inspectors.
Prompt and appropriate operator response to the June 23,1993 unanticipated single control rod

insertion was noted.
However, inspector followup to this event identified evidence of
programmatic weakness in the implementation of the preventive maintenance deferral process.
Specifically, it appears that many preventive maintenance activities originally scheduled for
implementation during the recent refueling outage were deferred by outage management without
~
completion of all required technical impact reviews. Your attention to this matter to ensure all
improperly deferred activities are promptly identified and properly evaluated consistent with your
station procedures is warranted.

On May 4,1993 you submitted a Licensee Event Report documenting a violation of technical
l
specification smveillance requirements for the Control Room High Efficiency Air Filtration
System. As discussed in the enclosed report, the successful completion of subsequent testing
established system operability during the period of time covered by the lapsed surveillance
activity. It is noted that this problem was discovered by your Quality Assurance Department
during the conduct of an audit. Such identification of the omitted surveillance test, along with
the corrective measures directed toward both procedural and supervisory review enhancements,
provides evidence of an active quality verification program and the proper safety perspective of
your corrective actions. Consequently, in accordance with NRC policy on the exercise of
discretion for enforcement sanctions, this violation is not being cited.
