NJ0025411 5-DAY REPORT - CLAIM OF UPSET CASE NO. 93-07-25-0328-44 In accordance with NJAC 7:14A-3.10, Public Service Electric and Gas Company (PSE&G) is submitting this 5-day. report for the exceedance of the daily maximum permit limit for Total Residual -

Chlorine (TRC) in the Station's cooling tower blowdown discharge to the Delaware River, DSN 461A. PSE&G reported this exceedance ,

to the NJDEPE Hotline and was assigned Case No. 93-07-25-0328-44 by Operator 14. The following information was provided during  :

this call:

i. Description of the discharge: Sodium hypochlorite

- location: Hope Creek Generating Station Cooling tower blowdown

Dependent upon investigation results. System will remain isolated until line is repaired.

Since the original call to the Hotline, PSE&G has reviewed the residual chlorine stripchart associated with the cooling tower i blowdown and determined that the discharge violation began at approximately 8:23 p.m. on July 24 and terminated shortly-before l 5:00 a.m. on July 25. The TRC concentrations for the period are -

estimated below. Final calculated TRC values shall be submitted {

The Station's NJPDES permit (NJ0025411) limits TRC concentration in the cooling tower blowdown to a monthly average concentration l of 0.2 mg/l and a daily maximum concentration of 0.5 mg/1.- A  ;

copy of the stripchart is included for your information as Attachment 1. A second copy of the stripchart has been included i as Attachment 2 and has been annotated to include information on 1 time, TRC concentration and other events and comments associated with the incident. Based on the stripchart, operating logs, and i interviews with personnel involved, PSE&G has prepared a chronology of events (Attachment 3) which details the '

circumstances surrounding this incident. In support of this  ;

information', PSE&G is also submitting copies of relevant system  !

operating logs which provides the technician's description of the l incident (Attachment 4) and the results of TRC analyses performed during the period (Attachment 5).

The TRC violation was caused by a leaking flange gasket in the i sodium hypochlorite injection system used for the Station's circulating water system.. The leaking gasket allowed sodium '

, hypochlorite to drain from the storage tanks to the chemical  !

building floor drain which discharges to the 1C lift station i l

NJDEPE 3 JUL 3 01993 NLR-E93228 servicing the Station's Low Volume and Olly Waste (LV&OW) system.

This floor drain is also used for other low volume wastestreams, including a continuous chlorine analyzer, and must be left open to prevent water from flooding the room. The 1C lift station is designed to operate based on level controls and automatically transferred the chlorinated water through the LV&OW system to the cooling tower blowdown line. The oscillations on the stripchart are the result of the 1C lift station cycling on and off based on water level.

Chemistry Department personnel inspect the sodium hypochlorite injection system once per shift as part of the technician's routine rounds. Leaks or problems identified during these rounds are reported to supervisory personnel and prompt action is taken to prevent discharges of chlorine or other chemicals to the i environment. In this case, upon discovery of the leaking gasket, the technician immediately isolated the source of the leak and took actions to plug the floor drain. Please refer to the chronology of events (Attachment 3) for other actions taken by the technician to reduce effluent TRC concentrations and to notify supervisory and response personnel.

Discovery of the TRC violation was hampered due to a plannou electrical outage in the 13KV loop which services the chlorination and dechlorination systems as well as the remote (Chemistry Department) indication of effluent TRC concentrations. <

The chlorination and dechlorination systems had been taken out-of-service prior to the electrical outage. Even after discovery of the TRC spikes on the stripchart, the technician thought there was a problem with the analyzer because the chlorination system was not operating. It was during the cleaning and verification of the analyzer that the leak was identified.

On July 26, 1993, a work activity was issued to replace the leaking flange gasket (Attachment 6). The work was completed the following day and the line was retested and found to be satisfactory.

PSE&G believes and maintains that it acted in accordance with NJAC 7:14A-3.10 which requires notification of any violation of a daily maximum permit limit to the NJDEPE. PSE&G also believes that based upon the regulatory definition of " Upset" contained in NJAC 7:14-8.2, the requirements for claiming an " Upset" in NJAC 7:14A-3.10(h), and the attached documentation, that all requirements necessary to claim an affirmative defense have been satisfied and that no NJPDES permit violation occurred.

23:15 Midshift tech begins his shift. He noticed effluent TRC alarm but knows CWS and SWS chlorination systems are shut down. He concluded false analyzer or computer reading due to power outage.

25 JUL 93 02:45 Midshift tech starts up ammonium bisulfite pumps during tour in preparation for startup of SWS chlorination system.

02:52 Midshift tech goes to blowdown effluent i sample house to sign chart and obtain j sample. He finds numerous spikes on stripchart. He can not explain source of spikes because chlorination systems ,

are shut down and dechlorination is operating. He flushes the analyzers of any accumulated debris.

03:02 Midshift tech obtains a grab sample of the effluent for TRC analysis. On the way to the lab, he stops at the CWS chlorination house to sign charts. Upon entering, he smells chlorine and finds hypochlorite leaking from the suction side of. injection pumps to the floor drain. He immediately goes to the tank isolation valves and isolates the source of the leak. r 03:15 Midshift tech calls the Senior Nuclear Shift Supervisor (SNSS) and Duty l Supervisor to ensure proper NJDEpE notifications are made and that Site '

Protection responds to chemical release. He then analyzes effluent sample with the results off-scale.

03:45 Midshift tech goes to ammonium bisulfite  ;

pump house, takes manual control of the l system and maximizes pump output (100%  :

stroke, 100% speed) He returns to the effluent TRC analyzers and watched meter  ;

drop from 2 mg/l to 0.5 mg/1. i i

03:50 Midshift tech returns to ammonium bisulfite pump house and starts second bisulfite pump. Upon return to effluent analyzers, meter reading is <0.001 mg/1.

04:00 Midshift tech calls to SNSS with updated information and reports inside to provide estimate of volume lost from '

tank (estiraate 300 gallons) .

04:45 Midshift tech returns outside for system tour.

05:00 Midshift tech signs stripchart and obtains effluent grab sample. Analyzers are runr.ing normally and agree with grab sample result of <0.05 mg/l (analysis at 05:09).

05:40 Midshift tech signs stripchart. All readings at <0.05 mg/1.
