SUBJECT:
NRC INSPECTION REPORT 50-482/93-14 (NOTICE OF VIOLATION)
This letter refers to the inspection conducted by Mr. G. A. Pick and the
inspectors identified in the attached report for the period of May 9 through
June 19, 1993.
The inspection included a review of activities authorized for
. your Wolf Creek Generating Station. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observations of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation includes two examples of inadequate.
,
transfer of Cycle 7 specific design information into the appropriate documents
and computer data bases.
The first example is of concern because it resulted
)
in violating a Technical Specifications surveillance requirement when
personnel failed to enter the appropriate axial flux difference limits into
the computer.
The second example cited could have resulted in a
nonconservative shutdown margin calculation because the table personnel
referenced was based on a higher rated thermal power than the current
100 percent rated thermal power. Due to inadequate communications, personnel
responsible for the implementing procedure were unaware that the core design
report tables prior to 200 effective full power days were based on a higher
rated thermal power.
Wolf Creek Nuclear Operating Corporation personnel discovered a mispositioned
valve that rendered one of two hydrogen analyzer trains inoperable.
Review by
licensee personnel identified no definite root cause; however, site personnel
promptly implemented corrective actions to prevent recurrence.
This violation
of Technical Specification 6.8.1.a will not be cited because the criteria
identified in paragraph Vll.B.2 of 10 CFR Part 2, Appendix C (Enforcement

Policy), were satisfied.

thermal expansion monitoring during the next outage .that requires heatup from

cold shutdown conditions.
