NRC INSPECTION REPORT NO. 50-62/93-02

'

This refers to the inspection conducted by C. Bassett of this office on

'

June 3 and 4, 1993 which was a continuation of the inspection documented in

NRC Inspection Report 50-62/93-01. The inspection included a review of

activities authorized for your University of Virginia Reactor (UVAR) facility.

At the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed report. A Confirmation of

'

Action Letter was also sent to you on April 30, 1993.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

i

and representative records, interviews with personnel, and observation of

activities in progress.

Based on the results of this inspection, four apparent violations were

identified and are being considered for escalated enforcement action in

accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy),10 CFR Part 2, Appendix C.

Accordingly, no Notice of Violation is presently being issued for these

inspection findings. The apparent violations involved:

(1) operating the

reactor without all of the required safety system channels operable, (2)

peforming maintenance on a safety system component without subsequently

verifying that the system was operable before it was returned to service,

(3) failure to have adequate procedures for performing troubleshooting and

maintenance, and (4) failure to follow a procedure.

Please be advised that

the number and characterization of apparent violations described in the

^

enclosed inspection report may change as a result of further NRC review.

An enforcement conference to discuss these apparent violations has been

t

scheduled for June 29, 1993. The purposes of this conference are to discuss

the apparent violations, their causes and safety significance; to provide you

~

the opportunity to point out any errors in our inspection report; to provide

!

an opportunity for you to present your proposed corrective actions; and-to

,

discuss any other information that will help us determine the appropriate

l'

enforcement action in accordance with the Enforcement Policy.

You will be

advised by separate correspondence of the results of our deliberations on this

-

matter. No response regarding these apparent violations is required at this

i

time,
