SUBJECT: NRC INSPECTION REPORT 50-445/93-22; 50-446/93-22 (NOTICE OF
VIOLATION)
This refers to the inspection conducted by Mr. W. B. Jones, and the inspectors
identified in the enclosed report, of this office, on April 28 through May 29,
1993. The inspection included a review of activities authorized for the

Comanche Peak Steam Electric Station, Units 1 and 2, facility. At the
conclusion of the inspection, the findings were discussed with you and those
members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.
Within

these areas the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
,
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the licensee were conducted safely and in
accordance with NRC requirements.
'
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation is of concern because a preventive
maintenance task was not performed on safety-related equipment for a period in
excess of twice the established frequency and a required technical evaluation

was not performed to assess the effect on the equipment.
Your staff indicated
that the safety significance of deferring the preventive maintenance task was
minor.
However, in assessing the overall significance of the violation we
noted that an April 1993 technical evaluation identified a programmatic
weakness with the preventive maintenance process which contributed to the
current deficiency and no corrective actions were implemented.

You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC's Public Document
Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget, as
required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
