NRC INSPECTION REPORT NO. 50-336/93-81
This refers to the announced electrical distribution system functional inspection conducted by
Mr. Leonard Cheung of this office on March 15 to April 2,1993. The inspection included a
review of activities authorized for your Millstone Unit 2 facility. At the conclusion of the
inspection, the findings were discussed with those members of your staffidentified in the
,
enclosed report. Additionally, the status of the inspection findings was discussed with
Mr. K. Shipman during a subsequent telephone conversation on May 18, 1993.
The purpose of this inspection was to determine whether the e' lectrical distribution system
(EDS) as designed, installed and configured at Millstone Unit 2 was capable of performing
its intended safety functions. In addition, an assessment of engineering and technical support
relative to the electrical distribution system was performed.
Areas examined during the inspection are identified in the report. The inspection was
directed toward areas important to public health and safety. Areas covered included the
design, installation, testing, and maintenance of the offsite power supply and onsite power
supply systems. Items covered included the_ emergency diesel generators, power
transformers, fast transfer scheme, circuit breakers, batteries and battery chargers. Within -
these areas, the inspection consisted of selective examinations of design calculations, relevant
procedures and representative records, installed equipment, interviews with personnel, and
,
observations by the inspectors.
Based upon the results of this inspection, ;wo of your activities appeared to be in violation of
NRC requirements. One of these activities involved inadequate corrective actions for
emergency diesel generator testing, as discussed in the enclosed Notice of Violation (Notice).
The violation is of concern because the corrective actions for a previous similar problem

identified by the NRC did not prevent it frora occurring. The other violation, pertaining to
_
the lack of acceptance criteria for the reset voltage for the degraded voltage protective relay
J
test procedure and use of inappropriate test instrumentation, was not cited because the
criteria of Section VII.B.1 of the Enforcement Policy were satisfied. Specifically, the
violation would r.armally be classified as a Severity Level V violation, and appropriate
corrective actions were completed promptly after it was identified.
You are required to respond to this letter and you should follow the instructions of the
enclosed Notice when preparing your response. In your response, you should document the
specific actions taken and any additional actions you plan to prevent recurrence. After
reviewing your response to this Notice, including your proposed corrective actions and the
results of future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.

The team findings raised several concerns in the design and testing of Millstone Unit 2
electrical distribution system. These concerns are: 1) diesel generator steady-state loading
calculations were not yet finalized; 2) diesel generator transient calculations were not verified
,
by test data; 3) the adequacy of housekeeping procedure for equipment temporarily stored m ~
j
the safety-related areas needs to be evaluated; and 4) voltage recovery capability of the
4160 V safety-related buses following a postulated LOCA condition when the post-LOCA

load is added to the normal house load causing the bus voltage to dip to 88% nominal
voltage.
We request that you provide to us, in writing, within 60 days of receipt of this letter, a
schedule for the resolution of the 13 unresolved items identified in the executive sumr .ry of

the inspection report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosure will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Pub. L.96-511.
