Box 220 St. Francisville, Louisiana 70775 SUBJECT: NRC INSPECTION REPORT NO. 50-458/93-10 This refers to the inspection conducted by Mr. W. F. Smith, and the inspectors identified in the attached report, of this office on March 14 through April 24, 1993. The inspection included a review of activities authorized for- your River Bend Station facility. At the conclusion of the inspection, the findings were discussed with you and those members of your staff identified in the enclosed report. Areas examined during the inspection are identified in the report. Within these areas, the inspection consisted of selective examination of procedures and representative records, inspections of installed equipment and system lineups, interviews with personnel, and observations of activities in progress. It was identified that your operators scrammed the wrong control rod during reactor flux tilting tests, contrary to the sequence prescribed in tSe test procedure. This was in violation of Technical Specification 6.8.1.d. The violation is not being cited because the criteria in paragraph VII.B.(2) of Appendix C to 10 CFR Part 2 of the NRC's " Rules of Practic9" were satisfied. This violation was of minimal safety significance because the rod was already inserted, the test was performed at a power level where a rod withdrawal or insertion anomaly would not result in an undesirable transient, and your staff took prompt and effective actions to correct the problem. In accordance with 10 CFR Section 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document Room.
