Dear Mr. Antony:

This refers to the routine safety inspection conducted by Mes rs. M. Dapas,

R. Bywater, D. Butler, and T. Kozak of this of fice from March 30 to

May 31, 1993, of activities at the Prairie Island Nuclear Generating Plant,

Units 1 and 2, authorized by NRC Operating Licenses No. DPR-42 and No. DPR-60,

and to the discussion of our findings with Mr. M. D. Wadley and other members

of your staff at the conclusion of the inspection.

'

The enclosed copy of our inspection report identifies areas examined during

the inspection.

Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

)

interviews with personnel.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation cited in Item A of the enclosed Notice is

of particular concern because one individual adjusted four redundant reactor

trip channels out-of-specification without questioning the magnitude of

adjustment or informing appropriate staff and without any management oversight

or review.

In view of this concern, this violation was considered for

potential escalated enforcement. We think that your corrective action should

be broadly focused and comprehensive.

As part of your response to this

j

violation, we request that you conduct a review of maintenance and testing

j

activities to determine if other conditions exist where lack of an independent

or management review, or lack of required notification / recording of the

"as-found" condition could result in an impact on operational safety.

We

request that you report the results of this review in your response to the

violation.

With respect to Item B of the enclosed Notice, this violation was reviewed

against the criteria for a non-cited violation.

We concluded that your

planned corrective action as documented in the associated Licensee Event

Report was not sufficiently comprehensive to address all of the apparent

causes for this violation.

In addition, this violation is of concern bacause
