SUBJECT: NRC INSPECTION REPORT 50-313/93-04; 50-368/93-04

This refers to the inspection conducted by Ms. L. J. Smith, and the inspectors

identified in the enclosed report, of this office on March 7 through

April 17, 1993.

The inspection included a review of activities authorized for

your Arkansas Nuclear One, Units 1 and 2, facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examination of procedures

and representative records, interviews with personnel, and observations of the

activities in progress. The results of this inspection are documented in the.

enclosed report.

It was identified that, while your staff maintained a clear understanding of

the operability of the auxiliary electrical power. system components, the

failure to realize the applicability of Technical Specification 3.7.2.A

following the loss of 4160V Bus A-4 was a weakness. This was an isolated

occurrence and your staff took prompt and effective immediate corrective

actions.

Comprehensive actions to correct the problem have been planned.

It

is our understanding that Mr. J. D. Vandergrift, Unit 1 Plant Manager,

committed to ensure operator training was conducted on Technical Specification 3.7 requirements and that the need to refer to the text of the

Technical Specifications rather than relying on memory would be emphasized.

If your understanding of this commitment differs from ours, please contact us

immediately.

Based on the results of this inspection, certain licensed activities appeared

to deviate from commitments made to NRC, as specified in the enclosed Notice

'

of Deviation (Notice).

We are concerned because the failure to design for an environment that had the

potential for introducing condensate into the limit switch compartment was a

contributing cause for the inoperability of the emergency feedwater pump

i
