50-328 SEQUOYAH NUCLEAR PLANT (SQN) - AUTHORIZATION TO USE AMERICAN SOCIETY OF MECHANICAL ENGINEERS (ASME) BOILER AND PRESSURE VESSEL (B&PV)' CODE-CASE N-389 Enclosed is a copy of ASME Code Case N-389 that.was approved by ASME on July 25, 1983. TVA has reviewed the applications of the code case and plans to apply it in conjunction with its ASME Section XI Repair and Replacement Program by implementing ASME Section XI, 1986 Edition through the 1986 Addenda for the purpose of installing welded tube plugs in steam generator (S/G) tubesheets.'

TVA plans to remove two tubes from S/G No. 3 for the purpose of analysis of tube failures in order to take the appropriate corrective measures to

ensure the life and operability of the S/G. The tube pulling and subsequent tube plugging by welding are emergent work from the-in-service inspection of the SQN Unit 1 S/Gs. When the tube is removed,'al welded tube plug will be Installed employing automated welding techniques. This' method of welding is used for significant dose reduction as it eliminates the requirement for individuals to remain inside the high-radiation field in the S/G primary channel head. This method of welding also has been demonstrated to produce a quality weld to TVA and the third-party' inspector employed at SQN.
