Special Report SR-93-05 Unit 2 Vibration ~& Loose Parts Monitor Operability of Channel 8 Associated Tech Spect 3.3.3.10 Related PIP's:
PIP 2-M93-0425 Cause of Malfunction:
Channel 8 (2MIMT5170, S/G 2D) of the Vibration & Loose Parts Monitor (V& LPM) was declared inoperable af ter it was discovered to have a low signal level by Operations on April 13, 1993. WO 93029919 was initiated and channel investigated by IAE.

The cause of the low noise level is damaged or bad cable to the associated sensor or ' poor cable.

Instrument Vendor V& LPM manufactured by Rockwell International Generic Concerns:
The V& LPM has a history. of unreliable performance. Specifically, numerous sensor cable and termination problems have been experienced due to their susceptibility to damage during installation or nearby maintenance activities.
Compensatory Heasures:
No compensatory measures are planned 'or required.
Plans for Restoring Operability:
Channel 8 of the V& LPM will be repaired under WO 93029919 during the next Unit 2 refueling outage (2EOC8) when cables and terminations are accessable.
Future Plans for V& LPM:
NSM's MG-12096 (Unit 1) and MG-22096 (Unit
- 2) are scheduled for completion during lEOC9 and
- 2EOC9, respectively.
Implementation on these :.bM ' s will begin during lEOC8 and -2EOC8, respectiveli.
These NSM's will install a completely new system and address system failure trends.
