SUBJECT: NOTICE OF VIOLATION (NRC INSPECTJON REPORT NO. 50-293/93-06
This refers to the safety inspection conducted by Messrs. J. Macdonald, A, Cerne, and D. Kern,
of this office on March 14 to April 26,1993 at the Pilgrim Nuclear Power Station, Plymouth,
Massachusetts. Areas relevant to the health and safety of the public examined during this
inspection are described in the enclosed report. Our findings were based upon observations of
performance and independent evaluations of safety systems and quality records. The preliminary
results have been discussed with you and other members of your staff at the conclusion of the
inspection.
Initial outage activities have been well controlled. Notable improvements have been observed
in the performance of refuel floor performance. Institution of dedicated fuel handling crews

supervised by senior reactor operator licensed personnel and refuel bridge equipment and
c
software upgrades were positive initiatives. The loop or train approach to the outage has been
effective. Inappropriate actions by a technician during undervessel work caumd an unplannetl
entry into a Technical Specification limiting condition for operation. Subsequent corrective
actions were promptly implemented.
However, based on the results of this inspection, certain of your activities appeared to be in
violation of NRC requirements, as specified in the enclosed Notice of Violation (Notice).
Specifically, the pressure and teraperature limits for the reactor vessel bottom head as defined
by Technical Specifications were exceeded during the March 13,1993 reactor depressurization
and cooldowr. Adequate controls were not established to ensure this condition was identified
and properly evaluated in a timely manner. Subsequent engineering evaluation determined '
significant safety margin existed throughout the event. Additionally, the failure to establish

adequate design controls during the 1902 mideycle outage installation of voltage vgulating
transformers resulted in the associated input breaker trip setpoints being improperly established
which in turn caused the unanticipated deenergization of the Y3 and Y4120 volt busses during
the March 13,1993 event.

enclosed Notice when preparing your response. In your response, you should document the
specific actions taken and any additional actions you plan to prevent recurrence. After reviewing
your response to this Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is necessary to
ensure compliance with NRC regulatory requirements.
The responses directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Pub. L. No. 96.511.
