NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-289 Three Mile Island Nuclear Generating Station - Unit 1 License No. DPR-50 During an NRC inspection conducted on March 28,1993 - May 10,1993, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:
10 CFR 50, Appendix B, Section V, states that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructicas, procedures or drawings.
Contrary to the above, on September 8,1989, the licensee failed to adequatdy establish Drawing No. 5391-839-005 to modify the train 'A' and 'B' heat sink protection system cabinets. This resulted in a miswiring during the 8R refueling outage in 1990, and an inadvertent partial actuation of the emergency feedwater system in January 1992.
This is a Severity level IV violation (Supplement I).
Although 10 CFR 2.201 normally requires you to submit to this office, within 20 days of your receipt of this Notice, a written statement of explanation, we note that this violation has been corrected and those actions were reviewed during this inspection. Therefore, no response with respect to this matter is required.
