Docket No. 50-346

Centerior Service Company

ATIN: Mr. Donald Shelton

Vice President

Nuclear - Davis-Besse

c/o Toledo Edison Company

,

300 Madison Avenue

Toledo, Ohio 43652

Dear Mr. Shelton:

This refers to the inspection conducted by Messrs. R. K. Walton and

E. R. Duncan of this office, and by Mr. J. B. Hopkins of the Office of Nuclear

Reactor Regulation on April 19 through April 21, 1993. The inspection

included a review of activities authorized for your Davis-Besse Nuclear Pcwer

Station. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed report.

The inspection was conducted in response to two unplanned transfer of reactor

coolant inventory events that occurred on April 3 and April 13, 1993, while

your Davis-Besse Nuclear Power Station was shutdown for refueling.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of a selective examination of

procedures, representative records, and interviews with personnel.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because it directly resulted

in the inadvertent transfer of about 13,570 gallons of water from the reactor

-

coolant system to the borated water storage tank. This violation is being

issued because your corrective actions for a similar event in December 1992

Jgg,/

were not adequate to prevent occurrence of this event.

Both the April 3 and

April 13 events are of concern because, during one or both events, a number of

weaknesses during shutdown activities were demonstrated concerning monitoring

I(

of important plant parameters, operator cognizance of changing equipment

i

status and its effects on interfacing systems, attention to detail, operator

knowledge of the decay heat removal system, and communications. We request-

that'you be prepared.to discuss these events at a management meeting in the

.

Region III office on June 3, 1993.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your resoonse.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

,

During this inspection, one additional activity appeared to be in violation of

'

NRC requirements. However, as described in the enclosed inspection report,

you identified this violation. Therefore, the violation will not be subject

.

'

to enforcement action because your efforts in identifying and correcting the

violation meet the criteria specified in Section VII.B of the " General

Statement of Policy and Procedure for NRC Enforcement Actions," (Enforcement

Policy, 10 CFR Part 2, Appendix C).

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, enclosures, and your response to this letter will be placed in

the NRC Public Document Room.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this inspection.
