NRC INSPECTION REPORT 50-528/93-15, 50-529/93-15, AND 50-530/93-15

This refers to a routine announced inspection conducted by Messrs. D. Acker

and S. Sanchez of this office, during the period of April 5 through 9,1993.

,

The inspection covered activities authorized by NRC License Numbers NPF-41,

NPF-51, and NPF-74. At the conclusion of the inspection, on April 9, the

"

inspectors discussed the findings with members of your staff identified in the

enclosed report.

Areas examined during this inspection are described in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations by the

inspectors.

Based on the results of the inspection, certain of your activities appeared to

be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violations involved failure of your personnel to

.

ensure that:

required changes to drawings, procedures, and the Updated Final Safety

'

'

Analysis Report were identified and issued during performance of a

design change and

required tests of design changes were adequately performed.

In particular, we note that a system engineer and two engineering supervisors

approved an improperly completed design change package. We are, therefore,

l

concerned about the adequacy of engineering review and approval of retests for

i

design changes.

You are required to respond to this letter and should follow the instructions

spetified in the enclosed Notice when preparing your response.

In your

response, you should document the specific action taken and any additional

'

actions you plan to assure that design changes are properly tested and

affected documents properly updated. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.
