TURBINE BYPASS MALFUNCTIONS Enclosed is our Technical Review on Turbine Bypass Malfunctions. This review documented occurrences of tarbine bypass (steam dump) system malfunctions, inadvertent opening or c Nsing of turbine bypass valves, and the resultant effect on plant startep, operation, and shutdown.

Searches on the NUDOCS and SCSS database revealed 140 events relating to this issue. The study revealed that most bypass system failures result in a plant transient involving actuation of engineered safety features (ESF) systems.
