HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JUNE 8. 1990 Indian Point 2 The licensee performed the staff requested visual inspection of the lower interior surfaces of SG #24 by using a TV camera lowered into the annulus The results between the tube bundle wrapper and the steam generator shell.
of this inspection were recorded on video tape and reviewed by NRC staff personnel. Based on its review of this video tape, the staff concurred with the licensee's conclusion that the lower portions of the steam generator shell were essentially free of surface pitting. Therefore, the licensee is planning to begin heatup of the plant on approximately June 10, 1990 and expects to synchronize the generator to the grid on June 16, 1990.
A group of visitors from the Soviet Union visited Indian Point 2 on June 5, 1990. The visitors are members of Working Group 2 of the US-USSR Joint Coordinating Comittee on Civilian Nuclear Reactor Safety.
The purpose of the site visit is to acquaint the visitors with a U.S.-designed PWR facility.
The Comittee-to Review Generic Requirements is planning to visit Indian Point Units 2 and 3 on August 1-3, 1990. This is expected to be a routine visit during which the Comittee plans to tour the plant, including the E0F, and meet with utility management and an operating crew.
U.S.-USSR Cooperation: Peaceful Uses Agreement On June 1, Presidents Bush and Gorbachev signed a five-year agreement "on Scientific and Technical Cooperation in the Field of Peaceful Uses of Atomic This was initialed by V. Konovalov, USSR Hinister of Nuclear Power Energy."

l House as a guest of Secretary of Energy Watkins.
Cooperation.in nuclear reactor safety, administered by NRC, is one of the four components of the Agreement. The substance of this program will be unchanged from that now in effect. (The other three components of the Agree-ment cover nuclear research administered by DOE.)
An NRC Team Visit to the URENCO Enrichment Plants During the week of June 4, 1990, an NRC team of safeguards, safety and security specialists visited three European uranium enrichment plants operated by URENCO Ltd. The three plants are: the Almelo Plant in the Netherlands, the Capenhurst Plant in Great Britain and the Gronau Plant in West Germany. The purpose of the.
visit was to identify the safeguards and safety concerns related to operation of centrifuge enrichment plants. This information is needed in the development of the licensing requirements and guidance for the proposed Louisiana EnergyThe Services (LES) enrichment plant which is to be built in Homer, Louisiana.
LES plant will be the nation's first privately-cwned facility for providing nuclear fuel enrichment services.
Cintichem, Tuxedo, NY On May 30, 1990, a meeting regarding the decommissioning of Cintichem facilities was held with the New York (NY) Department of Labor. Understandings reached at the meeting form the basis for drafting an agreement on what should be included in the decomissioning, decontamination criteria to be used by the licensee, and arrangements for and conduct of verification surveys. NRC attendees included representatives from the Office of Nuclear Material Safety and Safeguards, Office of Nuclear Reactor Regulation, and Region I.
New York was represented by the State Departments of Labor, Health, and Environmental Conservation.

l Grand Gulf Unit 1; Waterford 3; Arkansas Nuclear One, Units 1 and 2 Entergy Corporation advised the staff on June 6, 1990, that the Securities and Exchange Commission (SEC) approved the consolidation of operation management for Grand Gulf, Unit 1 Waterford 3, and Arkansas Nuclear One, Units 1 and 2 under a single company, Entergy Operating Inc. (E01). The staff had previously reviewed and approved the consolidation by issuing license amendments subject to official implementation by June 12, 1990.
Approval by the SEC and public service comissions was necessary before official im)1ementation. Entergy Operations will also serve as licensing agent for t1e Grand Gulf, Unit 2 Construction Permit, which is expected to be officially withdrawn this year. A letter from Entergy Operations is expected shortly to make the implementation official. The staff in NRR has consolidated licensing management of these units under Project Directorate IV-1.
Meeting With FAA On May 31, 1990, members of the Division of Reactor Ins)ection and Safeguards met with an Aviation Safety Inspector (Manufacturing), rAA to discuss NRC's policies and procedures'for identifying, investigating and disseminating information on counterfeit / fraudulent parts supplied by vendors to the nuclear industry.
FAA is in the process of setting up a system to accomplish the same purpose for FAA activities.

Generic Fundamentals Examinations-On June 6, 1990, NRC administered a Generic Fundamentals Examination Section-of the Written Operator Licensing Examination to approximately 200 candidates..

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending June 8, 1990 SAFEGUARDS AND TRANSPORTATION International International Atomic Energy Agency (IAEA) Physical Inventory Verification at the Babcock & Wilcox (B&W) Fuel Company On~ June 11, 1990, an International Safeguards Branch staff member will observe the physical inventory at the B&W Fuel Company in Lynchburg, VA. The staff member will also be present when the IAEA conducts their physical inventory verification starting on June 14, 1990.

Transportation Meeting on Westinghouse Titan Cask On June 5, 1990, Transportation Branch staff met with Westinghouse Electric Corporation (Westinghouse) and the Departnent of Energy (DOE) to discuss the preliminary design of the Titan spent fuel shipping cask. The Titan cask is being designed under contract to DOE as a legal weight truck cask for shipments of spent fuel under the Nuclear Waste Policy Act.
The cask has a capacity of.four. pressurized water reactor or seven boiling water reactor assemblies.
l Issues discussed included material properties and welding procedures for fabricating the cask body from titanium and efforts to incorporate titanium alloy in Section III of the ASME Code. An application for NRC certification is expected in January 1993.

Week Ending June 8, 1990 Division of Engineering National Research Council Panel on Coupled Systems The National Research Council's panel on the effects of coupled hydrologic, tectonic, and hydrothermal systems at Yucca Mountain, Nevada met at the U.S.
Geological Survey in Menlo Park, California, on May 30-31, 1990. This was the second meeting of the newly formed panel.
The suitability of Yucca Mountain for High-Level Waste (HLW) disposal is being evaluated by the Department of Energy. Dr. Edward O'Donnell represented the Waste Management Branch, Office of Nuclear Regulatory Research (RES) at this panel meeting which focused on the hydrologic system. There were technical presentations by the U.S. Geological Survey, the State of Nevada and its contractors, and the Deaartment of Energy 1
(DOE). Some of the topics discussed included the regional lydrogeology, the
~
local hydrogeologic framework of the Yucca Mountain area, stability of the i
Yucca Mountain area hydraulic regime in response to underground nuclear explosions, paleoclimates of the region, effects of past pluvial climates on the regional water table, and parameter variability in the hydrologic modeling of Yucca Mountain. The meeting concluded with a presentation by Jerczy Szysmanski, DOE, on his concept of a regional coupled model and its long-term effects. The next National Research Council panel meeting will be held July 26-27 to examine the geologic / seismic / tectonic system.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending June 8, 1990 Division of Operational Assessment (DOA)
~The Zion Diagnostic Evaluation Team arrived at the Zion Station on Monday, June 4, to begin two weeks of onsite evaluation activities. The entrance meeting for the team took place at 1:00 p.m., the first day onsite. The meeting was attended by the AEOD Deputy Director, Region III Administrator and CECO Corporate and Senior Station Management representatives.
The " Response Technical Manual" (RMT-90), NUREG-0150, Volumes 1 and 2, was published by IRB this week.
It contains the response technical procedures used by the Protective Measures and Reactor Safety Team members. This document is the foundation of the Protective Measures Workshop which is going to be held on June 18-22, 1990 at the Federal Emergency Management Agency's (FEMA) National Emergency Training Center. Each regional office received copies for use by the AE0D staff during the upcoming Fall training sessions on this subject matter.
On June 6, IRB staff attended a meeting of the Catastrophic Disaster Response Group Subcomittee on Federal Earthquake Response to discuss catastrophic response planning, forthcoming exercises, and new energy emergency support functions.
On June 4. IRB staff attended an Interagency meeting to discuss the comments of all federal agencies pertaining to revisions of the Environmental Protection Agency's Protective Action Guides. Additional meetings have been planned to l
resolve the issues raised by the comments.
Division of Safety Programs (DSP)
AEOD made a presentation to ACRS on the Accident Sequence Precursor (ASP) program status and results. The presentation focused on significant insights obtained from the ASP program from 1984 to 1988.

Office of Governmental and Public Affairs Items of Interest Week Ending June 8, 1990 State Programs Arizona Radiation Control Program Review Jack Hornor Region Y State Agreements Officer, with the assistance from John McGrath, Region I State Agreements Officer, began a review of the Arizona Radiation Control Program on June 4 in Phoenix, Arizona. The review will continue through June 15. Preliminary staff findings will be reviewed by Regional and Headquarters offices and the results of the review will be con-firmed in a letter to the State.
L Catawba Emergency Planning Exercise Richard Woodruff, Region II State Agreements Officer, participated in an emergency planning exercise at the Catawba Nuclear Station on June 7 in York, South Carolina.
Mr. Woodruff served as Government liaison on the site team; Election to American Academy of Health Physics Joel Lubenau, State Programs, has been elected to the office of Treasurer for the American Academy of Health Physics. Mr. Lubenau's term of office will begin at the Winter Meeting of the Academy in 1991.

Vital Records Program The Information and Records Management Branch (IRMB) staff visited the Federal Emergency Management Agency (FEMA) emergency relocation site and 4
updated the vital records inventory.
3 IRM has been advised by the AEOD Emergency ResponsL Coordinator that five additional emergency relocation sites will be iced to serve the five NRC Regional Offices. AEOD has expressed an intr. rest in IRM supporting these five new emergency relocation sites by providing and maintaining selected vital records. At this time, it is unclear which records may be required or the method that will be used to maintain the records at these sites.
AEOD will provide additional information to IRM about their plans for the new relocation sites.

The conference covered new approaches in mental health care, the increasingly important role of employee assistance programs in the work environment, and trends in care for the elderly.

v Office of Small and Disadvantaged Business Utilization and Civil Rights Items of Interest Week Ending June 8, 1990 Civil Rights Program The Office of Small and Disadvantaged Business Utilization and Civil Rights hosted the agency's Sixth Annual EE0 Counselor Training Conference May 22-25, 1990 in Hunt Valley, Maryland.
Conferees, totaling thirty, included Regional and Headquarters EE0 Counselors and a representative from each of NRC's EE0 Advisory Committees. The Conference began with opening remarks from the Director, OSDBU/CR, followed by a presentation from Mr. Douglas Bielan from the Equal Employnent Opportunity Commission. EE0 Counselor training was provided by Creative Communications i
LAssociates, a small business contractor.
Carolyn Swanson, Carol Harris and Barbara Williams from the Office of Personnel also contributed to the Conference by providing invaluable information on matters ranging from staffing to labor relations.
The Conference was considered by all to be highly successful.

An Order Imposing a Civil Penalty in the amount of $6,750 was issued June 6. 1990 to Tulsa Gamma Ray, Tulsa, Oklahoma. This action was based on violations that, in the aggregate, were considered to represent a l~
i breakdown in control of licensed activities. The violations occurred in
- 2) personnel radiation exposure evaluation and recordkeeping; 3) pract the following programatic areas:
- 1) job-site radiation safety radio-graphy device inventories; and 4) transportation of radiography devices..
The. licensee responded to the Notice in a letter dated February 22, 1990.
After careful consideration of the licensee's response, the staff has concluded that nine of the ten violations did occur as set forth in the Notice of Violation and Proposed Imposition of Civil Penalty. The original

proposed civil penalty has been reduced by 10%, because the licensee submitted additional information to show that one of the ten violations did not occur. (EN 89-116A) 2.
A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $160,000 was issued June 7, 1990 to Minnesota Mining and Manu-facturing Company. This action was based on seven violations relating to 3M's failure to test and evaluate the use of static eliminators by general licensees, failure to make the required notifications to NRC and to these-general licensees, and making a design modification to three models of static eliminator without prior NRC review and approval. The i
staff submitted this action for the Commission's review in SECY-90-172 which was approved by the Commission.

The civil penalty was escalated by 25%.

U.S. Testing Company, Modesto, California paid the civil penalty in the amount of $5,000. The action was based on a number of violations of NRC requirements designed to protect members of the public against exposure in excess of Part 20 limits. The violations included the failure to properly establish and provide surveillance of radiation boundaries during radio-graphic operations at a hospital. Nine Mile Point Two Region I safeguards inspectors are participating in a Regulatory Effectiveness Review (RER) at Nine Mile Point Nuclear Power Plant during the period June 4-8, 1990. Other team aarticipants include representa-tives from NRR/ Reactor Safeguards Branc1 and members-of the U.S. Army.
The purpose of the RER is to review the effectiveness of the safeguards systems and determine if the Nine Mile Point security program, as implemented, is meeting the performance objectives of 10 CFR Part 73.-
The review focuses particular attention on the methods and procedures employed to protect-equipment vital for the safe shutdown of the reactor.

Original design and testing problems resulted j
in substantial deficiencies in service water flow rates to critical safety 7

In addition, during followup to discovery of this problem the licensee valved out service water flow to both diesel generators during reactor operation.
The licensee's engineering evaluation of these matters is continuing. The regional staff will be-t proposing an enforcement action.
On June 5, 1990, the Director, Division of Reactor Safety, presented senior and reactor operator licenses at an award dinner in Decatur, Illinois.

Braidwood and Dresden Nuclear Power Stations From June 5, 1990, through June 7, 1990, a delegation representing a Taiwan utility company toured emergency preparedness (EP) facilities and discussed EP programs with Connonwealth Edison Company representatives. The three-member delegation was accompanied by representatives from the Illinois Department of Nuclear Safety. They visited-Dresden Station, the Emergency Operations Facility (EOF) in Mazon, Illinois and the Braidwood Station.

D. C. Cook Nuclear Plant On June 7, 1990, representatives of Region III and NRR management, met with the licensee in the Region III office to conduct a quarterly management meeting to discuss plant maintenance, material condition, and other items of mutual interest.

Medical Licensee Workshop Region III sponsored a one-day workshop for medical licensees in Downers Grove, Illinois on June 7, 1990.
Speakers included the Regional Administrator, NMSS and regional staff and a representative of a broad scope licensee. A) proximately 150 licensee ard State regulatory personnel attended the worksiop. The Region is compiling a list of regulatory issues that were identified as needing resolution. Tt,ese issues included definitions and interpretation of various Part 35 regulations.

Gulf States Utilities On Wednesday, June 6, 1990, Gulf States Utilities reported that one of the two duplicate com) uter systems used to activate the River Bend Station sirens failed to wor ( during a periodic siren activation test. When the sirens failed to activate, the licensee immediately switched to the alternate computer and a silent test was successfully accomplished. No other test requiring sounding the alarms was performed because the public had been previously advised of the exact time of the scheduled test, and the licensee did not want to cause public concern. The licensee notified FEMA, the state of Louisiana, and the affected Parishet,.
