Automatic Valve Corporation Novi, Michigan APPENDIX B N011CE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on January 29 through February 1,1990 at the Automatic Valve Corporation (AVC), it appears that p-certain of your activities were not conducted in accordance with NRC requirenants which were imposed on ycu through purchase order contracts with NRC licensees.
Criterion 11 of Appendix B to 10 CFR Part 50 states, in part, "... establish...a quality assurance program which complies with the requirements of this appendix.
n The program shall be documented by written policies, procedures, or instructions and shall be carried out...in accordance with those policies, procedures, or instructions...The quality assurance program shall provide control over activities affecting the quality of the identified structures, systems, and components, to an extent consistent with their importance to safety."
The AVC quality assurance manual (QAN), Revision B, dated July 13, 1989, is inadequate in the following areas:

Criterion I, " Organization," of Appendix B to 10 CFR Part 50, requires, in part, that the authority and duties of persons performing activities affecting safety-related functions of coniponents be clearly established and delineated in writing.
Contrary to the above, the AVC QAM does not reflect the present organization and does not clearly describe the authority and duties of persons currently performing safety-related activities.

Criterion IV, "Procurenent Document Control " of Appendix B to 10 CFR Part 50, requires that measures be established to assure that applicable regulatory requirements, design bases, and other requircments which are necessary to assure adequate quality are suitably included or referenced in the documents for procurement of material, equipnent, and services, whether purchased by the applicant or its contractors or subcontractors, Contrary to the above, the AVC QAM does not require that quality j
assurance requirenents received with custoner purchase orders be imposed, in turn, on their subvendors for the supply of materials such as steel, elastoners, and gaskets.

Criterion Vil, " Control of Purchased Material, Equipment..and Services," of Appendix B to 10 CFR Part 50, requires that measures be established to assure that purchased material, equipment and services conform to the procurenent documents. These seasures shall include objective evidence of quality furnished.
Contrary to the above, AVC accepted certified material test reports and certificates of conformances sup(90-01-04) plied by vendors withou verifying the validity of such docunents.
