This refers to the routine, unannounced inspection conductec

- by Kris 51embarger of. this office on March 16 and April 3,1990, of activities

at Zion Generating Station authorized by Operating Licenses No. DPR-39 and

DPR-48 and to the discussion of our findings with Mr. Glenn Trzyna and other

members of your staff at the conclusion of the inspection.

The inspection was performed to review the actions taken by the Zion Station

in response to Confirmatory Action Letter Rlll 89-021 that was issued as a

. result of the Zion Operator Licensing Requalification Program being rated-

. .

unsatisfactory in September,1989. The primary areas inspected were-(1) the

scope of the enhanced training provided and content of quizzes administered to

all licensed individuals at the Zion Station, (2) the attendance at the

training sessions, and (3) the make-up quizzes administered to individuals that

did not take the scheduled quizzes.

Following-the inspection activities Region 111 and licensee management

personnel met on April 23 and May 10, 1990 to discuss various aspects of the

Results: The Confirmatory Action Letter (CAL) indicated that enhanced

training based on those knowledge and performance deficiencies identified by

l

the Zion training staff, be provided to all licensed staff at the Zion

Station. Results of the inspection indicate that a broad scope of training in

the areas identified by Zion for enhanced training was provided. The quizzes

administered primarily examined the individual's knowledge in those areas

through the use of abnormal and emergency operating procedures.

Detailed

evaluation of the facility test items by the Region led to the conclusion that

the Zion quizzes did evaluate operator knowledge similar to that of the NRC

examination.

The CAL required completion of the enhanced training and examination by

March 31, 1990. Although the training, quizzes, and makeup quizzes were

scheduled for all licensed individuals prior to this date, not all license

holders attended training and/or were administered a quiz prior to this date.

As of March 31, 1990 s11 but six individuals had been examined. Makeup

quitzes were edministered to these remaining six operators by April 27, 1990.

Meetings were held on April 23 and May 10, 1990 during which G. C Uright,

Region 111 and T. Joyce, Zion Station, discussed the Zion Station impitmentation

of proceduras to ensure that operators not only attend requalification

lectures but also must demonstrate acceptable performance on quizzes according

to established standards.

purpose of inspection

The purpose of this inspection was to review the actions taken

in response to Confirmatory Action Letter Rlll 89-021 that was

issued as a result of the Zion Operator Licensing Requalification program

being rated unsatisfactory in September, 1989. The primary areas

inspected were (1) the scope of the enhanced training provided and the

Zion Station, (quizzes administered to all licensed individuals at the

content of the

2) the attendance at the training sessions and (3) the

make-up quizzes administered to individuals that did not take the

scheduled quizzes.

Results

a.

Scope of Training and Content of Quizzes

The Confirmatory Action Letter (CAL) indicated that enhanced

training and examination be administered to all licensed individuals

at the Zion Station by March 31, 1990. The remediation was to be

based on those knowledge and performance deficiencies identified by

the Zion training staff.

The inspection indicated that a broad scope of training in the areas

identified by Zion for enhanced training was provided.

The quizzes

administered primarily examined the individual's knowledge in the

areas of abnormal and emergency operating procedure application.

A detailed comparison by the Region of the quiz test items and the

'

NRC exam test items was made following a May 10, 1990 meeting

between Region 111 and licensee representatives.

This review led

to the conclusion that acceptable coverage of licensee identified

weaknesses had been met.

Training Attendance

.

The CAL required that enhanced training be provided to all licensed

[

individuals at the Zion Station during the period of January 1,

1990, through March 31, 1990. A review of the attendance records

indicated that twenty-six (26) of the eighty-four (84) licensed

individualsattheZionStationfailedtoatt,endallrequired

training sessions. This lack of attendance was considered a

weakness in the. licensee's requalification program particularly in

light of commitments made in the CAL. All licensee personnel who

missed this training were given the lecture notes for review and-

exams on the subject material. A new station procedure was issued

May 10, 1990, addressing attendance at requal lectures.

This

procedure was reviewed and found acceptable by the Region.

Make-up Quizzes

The CAL required that copies of all quizzes be provided to the NRC

in the Region 111 office within one week.following administration

with administration being complete by March 31, 1990.

the quizzes indicated that six (6) of the eighty-four (84) licensed

individuals had not been administered a regularly scheduled or

make-up quiz as of April 3, 1990. _All make-up quizzes were

completed as of April 27, 1990 and were provided to the region for

review.

Exit Interview

The inspector met with the licensee representatives denoted in Section 1

on March 16 and April 3, 1990. The-inspector summarized the purpose,

scope, and findings of the inspection, and the likely information content

of the inspection report.

The licensee acknowledged this information and

did not identify any information and did not identify any proprietary

information.
