test control program, if implemented as described, should enhance the control

and performance of surveillance testing.

In addition, Violation 313/8835-01,

involving a valve misalignment found prior to a containment integrated leak

SURVEILLANCETESTINGANDCALIBRATIONCONTROLPROGRt,MS(61725)

The purpose of this inspection was to ascertain the effectiveness of the

licensee's programs for controlling surveillance testing and calibration

activities.

.

The assessment of the surveillance testing and calibration control programs was

accomplished by reviewing the procedures and by discussing the aspects of the

program with licensee personnel. The documents that were reviewed are listed

in the Attachment to this report.

The surveillance testing program was undergoing revision during this

inspection. The inspector concluded that the revised surveillance testing

program for the portion presently in place, and the portions yet to be

implemented, should greatly enhance the control and timely performance of

surveillance testing. The calibration control program appeared to be well

implemented and smoothly running.

The licensee has experienced several instances where surveillance tests were

missed. The missed surveillances resulted in a notice of violation being

issued by the resident inspectors. The surveillance testing is controlled by

procedure 1000.009, "Surycillance Test Control Program," and the calibration

program is controlled by procedure 1000.115, " Preventive Maintenance Program."

The areas of weakness in the surveillance testing program appeared to be lack

of specific areas of responsibilities and misuse of the master test control

list (MTCL). The revised program assigns specific responsibilitiec for control

,

of the surveillance test prograin. The most noticeable improvement was the

creation of the position of " Supervisor, Surveillance Testing." The other

l'

major item was the purging of the MTCL so that it will become a viable tool in

'

identifying and scheduling surveillance tests.

The major comoonent of the surveillance and calibration activities is the

station information management system (SIMS). SIMS is a data base that

contains equipment information. The data base had been recently verified by

the licensee to be accurate. The licensee had performed a review to determine

the Technical Specification (TS) required surveillance testing and the non-TS

.

required testing and calibration. The surveillance test schedules were genereted

from the SIMS data base using the TADEM computer. The weekly testing schedules

.

were distributed to the appropriate discipline for performance.

'

Other changes to the surveillance test program included notification to the

responsible group of tests that will exceed the TS limit two days prior to the

overdue date. All surveillance tests, when completed, were reviewed by the

-

surveillance testing group for completeness prior to filing.

One long-term task is in progress. This task is to review each surveillance

test to verify that the test addresses the specific requirements of the TS and

produces the necessary results to satisfy the specific TS requirement. This

aspect of the program is expected to require 2 years to complete. The technical

adequacy of the tests had already been verified.

The inspector concluded that the revision to the surveillance test program will

result in a more positive control of the test program and should reduce the

probability of missed or overdue surveillance tests.

No violations or deviations were identified in the review of this program area.

3.

FOLLOWUP ON ITEMS OF NONCOMPLIANCE / DEVIATIONS (92702)

(Closed) Violation 313/8835-001,)" Valve Mispositioned for Cuntainment

Integrated Leak Rate Test (CILRT . During the pertormance of an independent

valve lineup walkdown prior to the start of a CILRT for ANO Unit 1, the

inspectors identified that Yalve ICW-32 was in the wrong position for the test.

.The valve was specified to be in the open position with a CILRT tag attached to

it.

It was discovered in the closed position with the CILRT tag attached.

The licensee imediately positioned the valve to the correct position and

reverified the test position of the remaining valves. The licensee determined

that Valve ICW-32 had initially been placed in the proper position, but had

subsequently been closed because of maintenance activities. The valve had not

been repositioned at the conclusion of the maintenance activities. The

corrective actions proposed by the licensee were to incorporate a specific

section on the CILRT into the general employee training (GET) and to revise the

CILRT tag to include the warning that shift supervisor approval is required

,

prior to changing a CILRT valve position. The inspector reviewed the GET

section addressing the CILRT. The tags had not been revised; however, the

'

licensee had issued two * Licensee Information Requests" to ensure that the

revisedtagswillbeusedduringthenextscheduledCILRT(L87-1338 for Unit 1

1

and LR 82-1339 forUnit2).

The use of the revised tags will be verified during the performance of the next'

l

scheduled CILRT, which will be for ANO, Unit 2 in mid-1991.

,

The inspector had no further questions regarding this item. The item is

considered closed.
