ENCLOSURE 1 NOTICE OF VIOLATION L

South Carolina Electric and Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 l

During the Nuclear Regulatory Commission (NRC) inspection conducted on l

April 1 - 30, 1990, a violation of NRC requirements were identified.

In-accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990), the violation is iisted below-10 CFR Part 50, Appendix B, Criteria V, requires that " Activities affecting quality shall. be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with those instructions, procedures or drawings".

Contrary to the above, on April 9,1990, the licensee discovered that three electrical leads to the reactor building level transmitter, LT 1976, were lifted during maintenance activities in November 1988 which rendered the transmitter inoperable.

Post maintenance testing activities prescribed by surveillance test procedure STP 135.001, Post Accident Monitoring Instruments Channel Check, Revision 7, were not adequate in that the visual comparison it. requires can not differentiate between a deenergized transmitter and an operable energized transmitter.

Contrary to-the above, on April 12, 1990, no procedures were provided for terminating the battery feeder cable circuit, number EDE'32B, to B battery.

MRF 21595, which provided instructions, did not contain adequate specificity to ensure that the cables were terminated-correctly.

The battery was. connected to-the energized DC bus with reverse polarity resulting in damage to two battery chargers and a loss of all power to safety related train B.

This is a Severity level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, South Carolina Electric & Gas Co.

is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, V.C. Summer Nuclear Station, within 30 days of the date of the Of bb o

This reply should be clearly marked as a

" Reply tc a Notice of Violation" and should include for each violation:

(1) admission or denial of the violation, (2) the reason for the violation if.

admitted, (3) the corrective steps which have been taken and the results achieved. (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
