SPECIAL REPORT During performance of the' Unit 2 Diesel-Generator B '
< ' Operability Test, the output breaker. tripped on reverse-power while attempting to synchronize the Diesel Generator with the 4160 Volt Essential. Power bus. The root cause of
.the. trip was. operator error. The operator failed to adjust the output voltage high enough above the line voltage to prevent actuation of the reverse power relay. This start attempt was classified.as.an invalid failure per NRC Regulatory Guide 1.108. It should be noted that the trip on reverse power is bypassed in the emergency mode.of operation.

The operator involved-in this incident-was counselled by management and afterwards demonstrated his ability.to successfully parallel the Diesel Generator to the Essential Power bus.
