NRC Region I Inspection Report No. 50-333/90-02
This letter transmits the results of the routine inspection conducted by
Messrs.
W. Schmidt
and
R. Plasse
of
this
office on March 12 through
April 25, 1990 at the James A. FitzPatrick Nuclear Power Plant, Scriba, New
York. Mr. Schmidt discussed the inspection findings with you and other members
of your staff at the conclusion of the inspection.

The inspectors concluded that two violations occurred.
The first violation
involves the failure to perform an adequate survey prior to refuel floor work.
This indicates that your radiation protection supervisor did not fully under-
stand your desire and need for increased sensitivity to radiological work. As
,
we discussed during the April 9 Enforcement Conference in Region I, we believe
that the performance of the radiological control supervisors and the adequate
review by ALARA personnel are important and should ensure that adequate radio-
logical protection measures are used.
You are required to respond to this
violation.

The second violation involves the failure of routine surveillance testing to
demonstrate that the high pressure coolant injection (HPCI) system would per-
form satisfactorily in service. We are encouraged by your commitment to per-
form an injection to the reactor vessel following the outage. Performance of
this test should better establish the system operational and testing require-
ments.
Further, based on review of your LER 90-10 a response to this violation

is not necessary.
The problems that you have encountered with respect to the emergency service
t
water (ESW) system and our review of the system indicate that additional action
is required on your part to ensure that ESW is acceptable to meet its design
i
basis.
We will schedule a management meeting to discuss this situation .with

you prior to unit restart,
