During the weeks of March 5, 1990, and March 12, 1990, the NRC conducted a

requalification program evaluation at the Peach Bottom Atomic Power Station

-

Units 2 and 3.~The evaluation was conducted in accordance with NUREG-1021,

j

-" Operator Licensing Examiner Standards," Rev. 5, section ES-601, Administra-

tion of NRC Requalification Program Evaluations." The evaluation had-two

j

. purposes: (1) to evaluate' individual licensed operator competency; and (2) to'

!

evaluate the effectiveness of the licensed operator training program. At the

conclusion of the evaluation, findings related to the preparation and admini-

,

'stration of the requalification examination were discussed with those members

.

'

.of your staff identified in the enclosed report.

As a result of this evaluation,-the Peach Bottom Atomic Power Station Units 2

and 3 requalification program was determined to be satisfactory. The'NRC results

indicate that twenty of the twenty two operators examined passed all portions

of the NRC administered:requalification examination and all four crews that'-

?

participated in the examination performed satisfactorily on the simulator portion

l

of the examination. One reactor operator and one senior reactor operator. failed

o

the written portion of the examination.- For the senior reactor operator and

reactor operator who failed the_ requalification examination you are expected to

remove the individuals from licensed duties, complete remedial training and

.

readminister a complete wr.itten examination in accordance with your requalifi-

.

cation program. After the-individuals have been successfully remediated and

reexamined by the facility they may be returned to licensed duties.

It also appears that your administrative controls were not adequate to assure

l

that the requirements of 10 CFR 50.54 and 10 CFR 55.59 were met in the areas

of -(1) . assurance that properly qualified licensed individuals are assigned'

j

licensed duties and (2) performance of an annual operating test and comprehen '

l

sive requalification written examination every 24 months. As such, this office

i

is _ issuing Notice of Violations enclosed as Appendix A to this letter.

You'are requested to respond to this letter and you should follow the instruc-

tions specified in the enclosed Notice when preparing your response. We are

concerned that the two violations appear to result from your staff not fully

j

assessing the changes made to 10 CFR 50.54 and 10 CFR 55 in May 1987.

In your

response, please include your assessment that your requalification program

i

fully meets 10 CFR 55.53 (h) and 10 CFR 55.59.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this

letter and the enclosures will be placed in the NRC Public Document Room. The

responses directed by this letter and enclosed notice are not subject to the

clearance procedures of the Office of Management and Budget as required by the

paperwork reduction act of 1980, Public Law No.96-511.
