L Combined Federal Campaign During an avtards ceremony in Chicago on March 16, 1990, the NRC, Region 111 Office, was awarded the Gold Award by the Federal Executive I

Board in recognition of Region III's contributions to the Combined Federal Cam)aign. The Gold Award is the highest award presented and signifies t1at a per capita of $100 per employee was contributed to the 1989 campaign.

Irene Little, Director, Division of Resource Management and Administration, was on hand to accept the award for the agency. Additionally, Gen Roy, former Director, Division of Resource Management and Administration, was presented a separate, special award for-his many years of dedicated wor k in the Combined Federal Campaign.

His efforts enabled the agency to achieve its superior success year after year.

Cintichem CintichemInc.(licensee),amanufacturerofradiopharmaceuticals, announced l

today that they are going to decommission their SMW research reactor in Tuxedo NY The licensee is a subsidiary of Medi-Physics, who is a subsidiary.of Hoffm6n-LaRoche, a Swiss Company. The reactor has been shut down since February 9,1990 and ordered to remain shutdown by an NRC Order dated February 13, 1990. Up to this time, the licensee was formulating plans to repair leaks in a concrete tank and pool and leaks in a ventilation duct.

Hatch Units 1 and 2 The labor dispute between the ARC health physics technicians and their company is still underway, although 28 of the 117 contract HPs reported for work on April 3.

Initially, only 7 of the HPs elected to cross the picket line. Over the last weekend, there was considerable absenteeism on the part of other contract employees, notably electricians and pipe fitters working for SINCO.

The licensee has been using GPC personnel to provide HP coverage, generally licensed operators who have had special HP training and other personnel who previously had served as HPs. All have been working under supervision of currently qualified plant HPs.

The outage activities at Unit 1 are now estimated to be about 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> behind schedule as a result of the strike with an additional slippage of about 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> per day while the strike continues.

ECT Indication in Cold leg Steam Generator Tube Plug at McGuire Unit 1 Sample eddy current inspections, reconnended by B&W, performed during the current outage at McGuire Unit 1, revealed an indication in an SG tube plug installed on the cold leg side. The affected plug is a B&W rolled plug fabricated from heat 2848-2, which has been previously identified by B&W as being highly susceptible to PWSCC. This plug is being removed for APRIL 6, 1990 ENCLOSURE A

met +11urgical examination to determine the nature of the indication.

If the indication is confirmed to be a crack, this would represent the first known instance of PWSCC affecting either B&W or Westinghouse plugs on the cold leg side. A 100% sample inspection of hot and cold leg plugs has now been performed. The staff is continuing to monitor plug PWSCC performance and effectiveness of licensee programs to ensure plug integrity.

Meetings with Regions Between April 3 and April 10, 1990, meetings were held with each Region. The discussions addressed the status of the plants and the status of maintenance at each facility.

1 Meeting With NUMARC - Procurement On April 3,1990, DRIS staff met with the NUMARC Working Group on Nuclear Plant Equipment Procurement to discuss the status of the NUMARC initiatives on licensee procurement, dedication, and audit programs. NUMARC has scheduled an industry meeting the week of April 9,1990 to review in detail the initiatives with representatives from each utility to improve the industry's understanding of the initiatives and to request each utility to analyze the potential impact of implementation. The Working Group's plan is to present the initiatives to the NUMARC Board of Directors at the June 1990 meeting. NUMARC offered to meet with l

NRC after the June meeting to discuss the results of the Board deliberations and l

actions.

l OMB Clearance Obtained OMB has approved the information collection requirements contained in the Fitness-for-Duty Rule through March 31, 1993. The OMB approval was based on the understanding that the NRC rule conforms to the drug testing guidelines of the Department of Health and Human Services, and that the NRC rule will be revised to conform to revisions that HHS makes to its guidelines.

SAFEGUARDS AND TRANSPORTATION Domestic Transaction Data Reconciliation i

ApprovalwasgiventotheDepartmentofEnergy(DOE),ChicagoOperations Office, to examine all past Nuclear Materials Management and Safeguards System transactions with Monsanto Company Dayton, Ohio, a licensee, to reconcile material balance data on a DOE Project. DOE and the Nuclear Regulatory Commission routinely cooperate in this reconciliation process.

Tactical Response Teams Tactical Response Teams have now been deployed at the three Category I fuel facilities in accordance with new requirements. Tactical response and guard exercises, including the use of force-on-force techniques, have also commenced.

Twelve drills per year covering each shif t and all components of the security system are required by the recently published rule.

Meeting on TRUPACT-II Shipping Package (NUPAC) f On March 29, 1990, thestaffmetwithNuclearPackagin,Inc.lthickness to discuss _ planned corrective actions by NUPAC to reso ve wal discrepancies on completed TRUPACT-II' shipping packages. NUPAC intends to qualify a weld process to build up the base metal on fabricated units.

NUPAC also plans to fabricate additional new units without the wall s

thickness discrepancies.

Meeting on UF6 Cylinder Overpack On March 29, 1990, the Transportation Branch staff met with representatives of l

Nuclear Packaging, Inc., (NUPAC), to discuss a new concept for UF6 cylinder I

overpacks. The new overpacks would be constructed of' fiberglass inner and 1

outer shell, with foam insulation between the shells. Use of fiberglass could potentially increase the corrosion resistance of UF6 over)acks and extend their service life. At the meeting, the structural and tiermal properties of fiberglass were discussed, together with manufacturing methods, reproducibility i

.and quality control. NUPAC plans to conduct full-scale tests and to further study fiberglass material properties before an application is submitted to the NRC.

LOW-LEVEL WASTE MANAGEMENT IMPACTS-Below Regulatory Concern (BRC)

On March 26-28,1990, the Division of Low-Level Waste Management and Decomissioning s>onsored a workshop for NRC staff to provide an overview and demonstrate tie use of the IMPACTS-BRC computer code. The IMPACTS-BRC code is a generic radiological assessment code intended to be used by NRC toevaluatepetitIonstoclassifyspecificwastestreamsasbelowregulatory concern (BRC). The code has recently been upgraded and is expected to be released for distribution in the near future.

Staff from the Offices of RES, NRR, GPA, and NMSS participated in this training session.

HIGH-LEVEL WASTE MANAGEMENT Division of High-level Waste Management Quality Assurance Observation Audit l

TheDivisionofHigh-LevelWasteManagement(HLWM)staffobservedaU.S.

Department of Energy (DOE) audit of the Los Alamos National Laboratory l

(LANL)fromMarch 26-30, 1990, relative to the implementation of their i-Quality Assuranc.e (QA) programs.

LANL is responsible for nuclide migration, geochemistry, mineralogy, and petrology studies on the Yucca Mountain Project.

The DOE audit was considered to be effective. The DOE audit team found that the LANL QA program is now adequate to control quality-related activities, and LANL should proceed with implementing their program. The effectiveness of the LANL QA program in the areas of audits, surveillances and corrective actions could not be determined as they have not been implemented. The HLWM staff agrees with the DOE audit team evaluation of the LANL QA program and will issue a report detailing its findings.

'DRA and NMSS staff met with representatives of the Edison Electric Institute to discuss EE! views on the transportation of low level wastes from nuclear reactors to waste burial grounds. The main focus of the meeting was the unresolved issue of external radiation control for a package which loses its radiation shielding in a transportation accident.

Pilot Program Workshops on Medical QA Rule The DRA staff is conducting Pilot Program Workshops on Medical QA rule in the i

five regions as follows: RegionI(EastElmhurst,NY), March 29; Region!!

i (Elk Grove Village, IL), April 4; Reg (ion 111 (Atlanta, GA) ' April 6; Region IV (Dallas,TX), April 18;andRegionV San Francisco, CA), April 20. These workshops are designed to assist the licensees,'who have volunteered to participate in a Pilot Program, in developing their' respective programs. The Pilot Program, on a trial basis, would determine the impact and effectiveness of l

the aroposed rule on " Basic QA Program for Medical Use of Byproduct Material" whic1 was published for public comments earlier this year. The 90-day public comment period will expire on April 12.

In response to NRC's invitation, 44 Agreement States and 22 NRC medical licensees have volunteered to participate in the Pilot Program which will be conducted for 60 days in the months of May and June. A QA Inspection Team will visit the facilities and conduct ' mock j

audits' at 18 randomly selected locations during the Pilot Program. At the completion of the Program, Post-Test Workshops will be held in each region from.

i Southeast LLW Compact Commission Meeting r

Robert Trojanowski Region II attended the Southeast LLW Compact Commission meeting on April 5, 1990 in Savannah, Georgia. The Comission was briefed by the North Carolina LLW Management Authority on the status of siting a LLW dis)osal facility in the State.

Richmond County has filed suit against the Aut1ority challenging its selection as a potential host area. Wace/Chatham Counties are the other areas being considered.

l New Jersey LLRW Disposal Facility Siting Board Meeting L

Marlene Taylor, Acting Region I State Liaison Officer attended the New Jersey LLRW Disposal Facility Siting Board and Advisory ComIttee meeting on April 5, 1990 in Princeton, New Jersey. The Board met to consider and approve draft final siting criteria, criteria based siting methodology, draft final disposal method selection criteria and draft final disposal method selection procedures.

Emergency Planning Exercise Participation This week State Liaison Officers from Regions 11 and IV participated in emer-gency planning exercises at Brunswick, and South Texas plants, respectively.

National Council of Radiation Protection and Measurements Joel Lubenau attended the annual meeting of the National Council of Radiation Protection and Measurements on April 4-5, 1990-in Washington, DC.. The title of-this year's meeting was " Health and Ecological Implications of Radioactively Contaminated Environments" with a focus on Department of Energy facilities.

Office of Enforcement Items of Interest Week Ending April 6,1990 Significant Enforcement Action A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $3,750 was issued on March 30, 1990 to Northeast Nuclear Energy Company (Millstone). The action was based on the shipment of a package of radioactively i

contaminated equipment that had external radiation levels on a small portion of the surface of the package in excess of regulatory limits. The escalation and mitigation factors in the Enforcement Policy were considered and the base amount of the civil penalty was increased by 50% due to the prior notice of such an event which was provided by an Information Notice in 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $2,500 was issued on April 3, 1990 to the Veterans Administration Medical Center in Newington, Connecticut. The action was based on: (1)thelicensee relieving the Radiation Safety Officer (RS0) of his radiation safety duties without appointing, and seeking NRC approval of, a new RS0; (2) failure to provide sufficient training to personnel working in the nuclear. medicine department (3) failure to perform certain required surveys or retain records of other surveys; (4) failure by the Radioisoto)e Committee to meet as required; and (5) failure to perform required checks of t1e dose calibrator. The escala-tion and mitigation factors in the Enforcement Policy were considered and although mitigation of the base civil penalty was warranted for the licensee's prompt and extensive corrective actions, this was offset by the fact that the NRC identified the violations. No other adjustment to the civil penalty amount was appropriate.

The NRC and United States Testing Company (UST) settled the matter of two civil penalties that were proposed in September 1989. The first was for $100,000 at i

Severity Level I for violations of requirements:as to use of radiographers and assistant radiographers without completing required training, examination, and certification. The licensee argued that this penalty was based on incomplete information. Under the settlement agreement, this violation was accepted and UST paid the $100,000 civil penalty.

The second violation, also proposed for Severity Level I at $100,000 concerned performance of radiographic operations at a facility. UST argued that the operations were authorized by its license which permitted _ radiography at temporary job sites. The staff agreed that if those activities were in accor-dance with NRC requirements, they were conducted with reasonable assurance that public health and safety would be protected. Iherapeutic Misadministration On March 16, 1990, J.F.K. Medical Center, Edison,. New Jersey, reported a therapeutic misadministration to a patient undergoing brachytherapy treatment to the lung. The misadministration occurred when the brachy -

therapy seeds (84 millicuries of Ir-192) were dislodged from the treatment i

The circumstances surrounding this incident and the licensee's actions are being examined by Region I.

Evaluation of the incident is continuing, including obtaining an NRC consultant physician to provide an independent medical assessment of the patient's condition, prognosis, and changes that may be necessary to prevent similar occurrences.

Seabrook Follow-up inspection to assess indicated high vibration of the inboard exciter bearing, the flexible cou)11ng between the generator and the exciter, determined that one of tie two gridmember (spring) layers had not been installed around the coupilng hub. The missing gridmember was installed, and the licensee is reviewing the installation error. Onsite assembly and installation of this bearing during construction was accomplished by the turbine manufacturer (GE). This bearing's 5 mil vibration was the only anomalous vibration indication during the turbine roll to 800 RPM.

Troubleshooting and calibration of the turbine speed control circuitry identified no major problems. Minor adjustments were made. On initial turbine roll af ter the maintenance, a turbine trip occurred at 140 RPM at 5:55 a.m., April 4.

The cause was initially attributed to failure to restore an electrical sliding link opened for the maintenance. The licensee placed a hold on turbine testing pending resolution of the error.

NHY evaluation of the cause is in progress.

Calvett Cliffs Baltimore Gas and Electric commenced heatup of Calvert Cliffs Unit I at 6:00 a.m. on April 4, 1990. The unit was shut down in May 1989 to inspect for potential 1ressurizer heater penetration leakage (pressurizer i

heater penetration leatage had been discovered on Calvert Cliffs Unit 2).

i Both units have been shut down since that time under Confirmatory Action i

Letter due to continuing problems with procedural adherence, control of system status, and control of work activities.

_ Indian Point 2 Steam Generator Cracking j

Consolidated Edison. Company is continuing to inspect for and repair i

cracks.in the Indian Point Unit 2 steam generator shells. Weld repair of approximately a 17 foot long section of the No. 22 steam generator upper transition girth weld has been completed. Cracks in other sections of i

that weld and the upper transition girth welds in the other three steam generators are being ground out.

The scope of the inspections has been expanded, and cracking has been found where the feedwater ring support brackets are attached to the inside of the steam generator shells in all four steam generators and in a circumferential band approximately ten i

inches below the upper transition girth weld in the plate material that makes up the transition cone in steam generator No. 22. The feedwater spargers and thermal sleeves have been removed to allow inspection in the bore of the feedwater nozzles and extensive cracking has been found in the two nozzles inspected to date. Cracks also have been found in the. feed-water nozzle to pipe welds'in three of the steam generators. A material semple has been removed from the bore of one feedwater nozzle and from the plate material 10 inches below the transition girth weld for metallurgical evaluations.

Repair plans are being analyzed and qualified. A minimum of two to three weeks of additional inspection and repair activities are anticipated. NRC resident and specialist inspectors are following activities onsite. The licensee has committed to submit a written evaluation of the condition of the steam generators to NRC and the staff plans to meet with the licensee prior to plant restart to determine if the steam generators are acceptable for continued operation.

Duke Power Company On April 3, representatives of the Duke Power Company were in the Region II Office to meet with selected members of the Regional stcff regarding security performance improvements.

Emergency Exercise Brunswick conducted an emergency exercise on its evening shift April 3, L

1990, which included partial State participe. tion and full local partici-pation.- The FEMA-observed exercise has resulted in preliminary JEMA findings indicating problems with the Brunswick County Emergency Opera-tions Center facilities, breakdowns in the offsite selective signalling system (ring down telephone) resulting in untimely offsite notifications, and Emergency Broadcast System messages that were unclear. The NRC team observing the licensee's performance showed problems with offsite notifi-cation messages with respect to content and timeliness and failure to conduct adequate onsite accountability.

Senior Management Meeting Pre-Brief On April 4, the Regional Administrator, accompanied by selected Region 11 staff, was in Headquarters to brief the Director, Office of Nuclear.

Nuclear Fuel Services. Inc.

On April 5, representatives of Nuclear Fuel Services, Inc., were in the Region II Office to meet with members of the Safeguards Section regarding proposed revisions to the NFS Physical Security Plan.

HP Technicians Strike The Health. Physics Technicians strike at Region 11 nuclear power stations, now in its second week, has been diminished by returning and replacement workers. Some outage work has been delayed; however, no alleged unsafe practices have been identified during special health physics inspections at the more significantly impacted sites or by resident inspectors at l

other sites.

Catawba Unit 1 The licensee discovered that during operation in Modes 4 and 3, all four steam generator PORVs (one per steam generator) were isolated by shut motor-operated block valves. The licensee entered Mode 4 on April 2 and Mode 3 on April 5.

The PORVs' isolation was discovered when reviewing a computer-generated critical parameters checklist prior to shift. turnover.

During a subsequent review, the licensee discovered closed valves from

the nuclear service water system to the containment valve injection water system surge chambers. These valves are required to be operable'in Modes 1-4.

The licensee is investigating both of these occurrences and had suspended further startup activities on Unit I until a complete review of plant configuration and the administrative program is performed. The Region is augmenting Resident Inspector coverage so that 24-hour coverage can be maintained through plant startup.

7.

Yogtle Unit 1 The Incident Investigation Team completed its onsite review of the Vogtle incident and is now preparing its report. The team is ascertaining the reason for the 1 "A" Diesel Generator trip, which occurred during the March 20 event. The utility currently plans on shipaing the suspect CALCON pressure and temperature switches to Wylie La> oratories for indepth analytical testing. The pneumatic air quality for the Unit I diesels currently does not meet dew point criteria (approximately 60-80 degrees F, asopposedto35-50degreesF). Testing.is on hold until this issue is resolved with the diesel vendor. Estimated criticality is Tuesday, April 10, 1990.

Region II specialists are onsite monitoring the Diesel Generator testing activities. A status meeting between the NRC and the utility is being arranged for Monday, April 9.

Combined Federal Campaign During an awards ceremony in Chicago on March 16, 1990, the NRC, Region III Office, was awarded the Gold Award by the Federal Executive Board in recognition of Region III's contributions to the Combined Federal Cam)aign. The Gold Award is the highest' award presented and signifies t1at a per capita of $100 per employee was contributed to the 1989 campaign.

Irene Little, Director, Division of Resource Management and Administration, was on hand to accept the award for the agency. Additionally, Gen Roy, former Director, Division of Resource Management and Administration, was presented a separate, special award for his many years of dedicated wor k in the Combined Federal Campaign.

His efforts enabled the agency to achieve its superior success year after year.

University of Minnesota On April 3, 1990, Charles E. Norelius, Director, Division of Radiation Safety and Safeguards, and selected regional staff conducted an Enforcement Conference with licensee management to discuss the findings of a recent inspection, the licensee's corrective actions and NRC enforcement options.

The item of major concern was the management's slow response to staff identifie*J deficiencies which were attributed to lack of adequate HP staff.

The University is now proposing to add two members to the staff.

I On April 4, 1990, A. Bert Davis, Regional Administrator, and selected regional staff conducted an Enforcement Conference with licensee management to discuss the findings of a recent inspection, the licensee's corrective actions, and NRC enforcement options. The major issues related to preparation of shipping pa7ers containing wrong information and multiple radiation safety-pro)1 ems which led to shut down of operations.

The licensee requested further modifications to-their existing Order Suspending License. The staff is presently reviewing these proposed modifications to the Suspension Order and is evaluating all of the information to determine further appropriate enforcement sanctions.

LaSalle Nuclear Power Station i

On March 28,1990, Unit 1 scrammed from 100 percent power. An insulator i

on the B phase line near the 1 East Main Transformer blew apart causing a differential current trip of the main generator. This resulted in the main turbine stop valves closing, which in turn, generated the reactor scram. A theory is that a short occurred and the arc-over caused the insulator to blow apart. The reason for the arc-over is not known. The insulator was replaced and the Unit started up on March 31, 1990.

Quad Cities Nuclear Power Station i

On April 3, 1990, the site experienced an Unusual Event due to the loss of ENS, NARS, and all other commercial offsite telephone lines. During the event, communication was reestablished via the licensee's microwave phone system. The cause of the event was the tripping of the main circuit breaker for the fiber optic terminal, apparently due to the breaker being undersize. Comercial telephone lines were reestablished approximately 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> after the event.

Fermi 2 Nuclear Power Plant Region III Division of Reactor projects was asked to provide information to the Office of International Programs to respond to a request by the Canadian Government regarding Zebra Mussels and asbestos in the cooling towers at Fermi.

Point Beach /Kewaunee Nuclear Power Plants On April 3, 1990, Messrs. Edward Greenman, Director, Division of Reactor Projects, and Dick Knop, Chief, DRP Branch 3, conducted site visits to I

the Point Beach and Kewaunee Nuclear Plants. The visits included. tours I

of the plants and meetings with the plant managers, plant operators and other personnel of both plants.

Items discussed during the meeting at Point Beach were: refueling items, staffing issues, open item tracking programs and other items of mutual interest.

Issues discussed during the meeting at Kewaunee were: results of recent licensing exams, security events, SSFI findings and other items of mutual interest.

Perry Nuclear Plant On April 3,1990, at about 2:30 a.m. while operating at 100 percent power, the licensee declared both Division 1 and 2 Emergency Service Water Systems e

inoperable due to failures in support equipment. While testing the i

Division 1 Emergency Diesel, the associated emergency service water pump discharge strainer lost a gasket seal on a handhole cover. The water-spray through the seal opening " wetted" a number of emergency service water system support components.

With Division 1 and 2 Emergency Service Water Systems inoperable, the

" supported" emergency core cooling systems (RHR-A -B,-C & LPCS) were declared inoperable and the licensee declared an alert in accordance with APRIL 6. 1990 ENCLOSURE.N

3 their emergency operating procedures. With the declaration of an alert, the licensee activated their Technical Support Center (TSC) and Operations Support Center (OSC). In addition, the licensee entered the provisions of Technical Specification 3.0.3 while commencing repairs to the Division 1 Emergency Service Water Support equipment. At about 4:00 a.m. the licensee completed repairs to the control power transformer. At about 5:00 a.m. the licensee completed resistance checks on wetted support equipment and commenced a performance test of-the support equipment. At about 5:30 a.m. all Division 1 support equipment was declared operable and Division 1 and 2 ECCS were declared operable. At about 6:00 a.m. the licensee, with concurrence of state and local officials, terminated the alert.
