This refers to your letter dated Au9ust 16,1989, for an amendment to License No. 45 03550 01.
The following additional infomation is needed in support of your request:
In order to authorize Diane Wilkinson, M.D., to perfom dia9nostic studies in 10 CFR 35.100 and 35.200, we will need a copy of her board certifica-tion in Dia9nostic Radiology.
In order to authorize Dr. Wilkinson to perform lodine 131 treatment of hyperthyroidism, we will need Supplement B of form 313M completed by her preceptor demonstrating that her training and experience meets the requirements of 10 CFR 35.932 (10 CFR Part 35, enclosed).
Our review of your application will continue upon receipt of the above infomation.
Please provide two copies of your reply and reference Mail Control Number 252968, if we do not receive a reply from you within sixty (60) calendar days from the date of this letter, we shall assume that you do not wish to pursue your application.

i We would like to add Diane Wilkinson, M.D. as an authorized user for materials in 10 CPR 35.100, 35.200 and 35.300 as appropriate.
We would also like Dr. Wilkinson authorized for usage of I-131 in treatment of hyperthyroidism.
Dr. Wilkinson is licensed to practice in the State of Virginia, and was Board Certified in Diagnostic Radiology by she American Board of Radiology in June 1987.
Her previous experience with I-131 treatment of hyperthyroidism consists of 12 cases at Brigham Young Hospital in Boston, Mass, during 1987-89.
Attached is a check payable to the U. S.11uclear Regulatory Commission in the amount of $120 as an amendment fee in accordance l
with 10 CFR 170.31 7. C.
