Statement of Violation :
Technical Specifications (TS)- Section 6.8.1 requires, in part, that
written procedures shall be established, implemented, and maintained,
including the applicable procedures recommended in Appendix A of
Regulatory Guide (RC) 1.33. Revision 2 (February 1978).
Plant
Procedure OPCP03 ZA-0010, " Plant Procedure Compliance, Implementation,
and-Review," Revision 11.,and Plant Procedure OPCP03 10 0004, " Plant
Conduct of Operations," Revision 10 require strict adherence with
approved procedures and the use of controlled documents which have
been verified as the current revision with the latest changes when
. performing operating activities.
Contrary to the above, on September 13, 1989, three examples of
failure to follow approved procedures were observed:
(1) An operator
failed to follow safety related Procedure OPOP08 FH 0003, " Fuel
Transfer System *, Revision 0, Steps 7.1.8 and-7.2.9 during the
operation of the Unit 1 fuel handling building fuel transfer control
console in that the upender hydraulic pump was not turned on and off
during each fuel movement cycle, and (2) an operator failed to have an
approved, up to date copy of Procedure OPOP08 FH 0009, " Core
Refueling," Revision 4, at the fuel handling building control console
workstation, and (3) an operator failed to follow the precautions of
safety-related Procedure OPOP08 FH-0001, " Refueling Machine Operating
Justification," Revision 0. Step 4.3, during the operation of the
Unit i reactor containment building refueling machine in that
unsecured articles were left on the refueling machine.
This is a Severity Level IV violation.

The safety significance of each example is presented below.

The upender hydraulic pump is required to be turned off

wrong position. If the pump is left on, other procedure
steps would have to be performed out of sequence before

damage-could occur to the fuel transfer system.

Examole (2): - The two FCRs dealt with supervisor instructions and

communications between the core loading supervisor and

-the'FHB-refuelihg' operators. The'worWors were aware of
the changes in the FCRs and were using the revised
eithod of communicating. There was no impact on the
refueling operation due to this incident.

Exannie (3):
Since none of the items identified fell into the
refueling cavity, there was no impact on the refueling
or plant operation.
III.
Reason for Violation
The root cause of this violation is lack of attentiveness to

procedural compliance requirements by members of the operations staff.

' the following actions were taken with r*5ard to examples 2 and 3,
respectively:

When the inspector pointed out to the duty operators that the Fuel

Handling Building working copy of procedure OPOP08 nl.0009, accre
Refueling" did not have two Field Change Requests (FCRs)
incorporated into the procedure, the duty operators immediately
replaced the working copy with an up to date copy containing the -
two FCRs.
2.. When the inspector pointed out to the duty operator that there
were articles on the refueling eachine that were not tied down in
accordance with the procedure, the articles were immediately
removed or tied down, accordingly.

procedure compliance policy with regard to employee understanding and
management enforcement. The review is to be completed and an action
plan for enhancing procedure compliance is to be developed by
December 31, 1989. The examples of failing to follow approved
procedures described in this subject violation are simi,lar to the
events described.in the referenced LER. Therefore, this violation
will be reviewed concurrently with Unit 1 LER 89 017 when developing
an action plan for the station's procedure compliance policy.
