. o Appendix NOTICE OF VIOLATION The Mead Corporation License No. 34-06188-07 As a result of the inspection conducted on March 11, 1982, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified:
Conditions 7F, 7G, 7S and 9F, 9G, 95 of your license limit your possession of strontium-90 sealed sources to certain specified manufacturers and model numbers for use in certain specified source holders and/or gauges.
Contrary to these requirements, on the date of the inspection, March 11, 1982, you possessed a sealed source of strontium-90 in a physical form and for a use not authorized by your license. Specifically, you possessed a sealed source of 15 mci strontium-90 manufactured by a Company other than those authorized by your license and housed in an Overhoff and Associates device. Your license does not authorize use of Overhoff and Associates devices.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this of fice within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be. submitted under oath or affirmation. Consideration may be given to extending your response time-for good cause shown.
