This refers to the routine safety inspection conducted by Mr. J. R. Mu11auer of this office on March 4,1982, of activities authorized by NRC Byproduct Material Licenses No. 13-02047-02 and 13-02047-03 and to the discussion of our findings with you, and several members of your staff at the conclusion of the inspection and telephone conversation with Charles Giffith on February 15, 1982.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.

A written response, submitted under oath or affirmation, is required.

This letter also refers to the telephone conversation between Charles Griffith and James R. Mu11auer on February 15, 1982, concerning your teletherapy calibration procedures.

Specifically, it was determined that your monthly spot check measurements do not include a correction for temperature and pressure. This correction factor should be included in accordance with ANSI standard N449.1-1978 " Procedures for Periodic Inspection of Cobalt-60 and Cesium-137 Teletherapy Equipment."

In your response to this letter, please confirm that the correction factor will_be' incorporated into your calibration procedures.

m o 41982 This inspection also included a measurement of the output of your tele-therapy equipment. Our measurement was made using a Victoreen Model 570 Condenser R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 55 centimeters and the field size used was ten by ten centimeters. After applying a correction factor for time error, our measurement of the output of your teletherapy equipment was 97.8 roentgens per minute. The most recent spot c. hock of your teletherapy equipment, conducted on February 17, 1982, reported a measured output for the same exposure parameters of 98.9 roentgens per minute. Applying a correction factor to account for 13 days decay, the value calculated from the February 17, 1982 spot check becomes 98.4 per minute.

Based on the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.

In addition to the above areas, the inspector examined actions described in your letter dated September 13, 1979, regarding apparent items of noncompliance found during our August 28, 1979 inspection. We have no further questions regarding these matters.

The responses directed by this letter (and the accompanying Notice) are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this inspection.
