Further explanation of question areas is as follows: Item 1 The flange of the reactor vesset head con $ining the stud holes,shows con-siderable rust on aLL the carbon surf aces white the sheLL areas protected by the inert gas purge during storage are relatively rust free. The rusted i areas are being cleaned. It em 2 The fillet weld connecting the CROM and instrument nozzles to the interior portion of the reactor vesset head,contain the f ollowing conditions on about 101 of the welds: ~ ..
- a. excessive grind marks into the housings
- b. weld overlay on housing surf aces
- c. cavities (incomplete welded areas)at the edge of the weld to the
- CROM housings and the edg e of the weld to the cladded sheLL surf ace
- d. under poor enviromental condstions.the site performed an inf ormatio Liquid penetrat test on sample welds with the f ollowing results :
- 1. Instrument nozzle M97, seat weld contained an 1/2" Linear i
indication simiiar to a hairline crack
- 2. CROM nozzle 560 - 1/8" Linear indication Additionally,the following conditions exist to the housings or the guide cones:
- a. heavy nicks and scratches on the outer surf aces of the housing
- b. some guide cones contain many ground areas - inside and outside of the cone item 3 -

CE resolution: The rust and dirty condition of the flang e containing the stud holes has been cleaned and is acceptable. No weld overlay was obs erved on the housing. Housing No. 60 had an area of incomplete weld which will re-quire blending and P.T. testing at a later date. (c) Cavities at the edge of the weld to the CROM housings and the edg e o f the weld to the cladded s hell surf aces. . . . . . . . Cavitics observed in no case exceed 1/32" deep and are acceptabic. CE resolution: Area of upset metal on the outer surf ace of the reactor vessel closure head flange will require blending and Liquid penetrant t esting . These area were covered with spraytat and could not be seen during the previous work.
