?PPE*tDIX A 30TICE_OF VIOLATIO:t Boston Ediron Cormany Docket !Io. 50-293 Pilgria :tuclear Power Station Unit 1 License he. !TR-53 As a result of the inspection concucted on August 31-September 4. and Septeeber 2C-October 2,1931, and in accordance with the Interin Enfortenent Policy, 45 FR 66754 (October 7,1963), the following violations were identified:

A.

Technical Specification 6.13 requires that each area in which the intensity of radiation is treater than 100 rrem/hr be conspicuously posted as a High Radiation Area.

Contrary to the abcve, on Septecter 2S,1981, the reactor drywell exhibited seneral area radiation intensities ranging from 100-300 rrer/hr and neither the area nor the snain access point to the area were posted as a liigh Radiation Arca.

This is a Severity Level IV Violation (Supplecent IV).

B.

Technical Specification 6.11 requires that procedures for radiation protection be adhered to for all operations invohf6,9 radiation exposure.

Procciure Ho. 6.1-022 Radiation Ucrk Pernit (RW), Revision 7. Section Y.A., requires that individuals working under the control of an P.WP follow all instructions on the WP. % P No.81-963, Overhaul Tools for Outage, dated July 17. 1981, requires cloth hoods to be worn while working. Section G of this sare procedure requires that all personnel entering or leaving a work area under the cor, trol of an RWP sign in on the access control thest indicating nw:e, tite in and out, and rocket desir:eter reading in and cut.

Contrary to the above:

1.

On September 2,1981 three out of four individuals working on the refueling floor under RVP Ho.81-963 with caterial contaninated to 10.000 dpm/100 cn2 (beta /gama) did not wear cloth hoods.

2.

On June 1 and 2, 1931 an individual entered the access to the drywell personnel airlock, an area controlled by RWP llo.

J 01E-1, and did not sign in or out on the access control sheet.

This is a Severity Level V Violation (sucplerent IV).

Pursuant to the provisions of 10 CFP. 2.201, the Coston Edison Corpany is hereby rquired to subeit to this office within thirty days of the date of this Notice, a written stateacnt or explanation in reply. including:the corrective step (1

corrective steps which will be taken to avoid further violations; and (3) the date when full coup 11ance will be achieved. Under the authority of Section IP2 of the Atoutc Energy Act of 1954, as acended, this response shall te sutaitted under oath or affirration. Vhere tood cause is shown, consideration will be given to extending year response tine.
