April 9, 1976 by Messrs. D. Neely and J. Strecter of our Region I office
in King of Prussia, Pennsylvania, of activities authorized by NP.C License
Nos. DPR-5 and DPR-26, and to the discussions held by Messrs. D. Neely
and J. Streeter with Messrs. W. Stein, E. Kessig and other members of
your staff on April 9, 1976. This also relates to the following:

The letter of September 13, 1976, from Mr. Hauspurg to the
Director, Office of Inspection and Enforcement, transmitting
the check for Twenty Thousand Eight Hundred and Fifty Dollars
($20,850) in full payment of the civil penalties imposed by
.
the August 23 order.

The letter of September 27, 1976, from the Director, Office of
Inspection and Enforcement, acknouledging the payment of the
civil penalties and documenting licensee. commitments mado during
the telephone discussions on July 15 and 16.
The area examincd during this special inspection was the health physics
program and was a result of the April 5,1976, exposure of a worker in
excess of the quarterly limit set forth in 10 CFR 20.101(b) . The inspec-
tion consisted of selective examinations of procedures and representr.1ve
records, interviews with personnel, observations, and radiation measurements
by the inspectors. The Items of Noncompliance that resulted from this inspec-
tion were docu=ented in the Notice of Violation sent to you on June 21,
1976.

In accordance with Section 2.790 of the NRC's " Rules of Practice", Part 2,
Title 10, Code of Federal Re3ulations, a copy of this letter and the enclosed

inspectio_n report will be placed in the NRC's Public Document Room.
If
this report contains any infor=ation that you (or your contractor) believe
to be proprietary, it is necessary that you make a written applica:1on within
20 days to this office to withhold'such information from public disclosure.
Any such application must include a full statement of reasons on the basis
