1. Take appropriate action to place the plant in a safe condition in accordance with emergency operating procedures, Tech. Specs., and annunciator response procedures.
2. Check wind direction and speed on panel H11-P658 to determine location of Emergency Operation Facility (EOF).

4. Make the following announcement over the PA:

ATTENTION, ATTENTION: A SITE AREA EMERGENCY EXISTS.

INITIATE SITE AREA EMERGENCY PROCEDURES. REPORT TO THE RALLY POINT LOCATED AT (ENVIRONMENTAL /5 KILLS TRAINING)

BUILDING BEFORE LEAVING SITE. EVACUATION ROUTE WILL BE

(@RTH/SCtlTH/EITHER DIRECTION) ON U.S. 1 HIGHWAY.

(@RMAL / AL. TERNATE ) E.O.F. WILL BE ESTABLISHED.

Sound the Gai-Tronics tone for Site Area Emergency and repeat announcement.

5. Check control room instrumentation to determine status of plant as to whether or not a General Emergency should be declared. Notify Shift Foreman of findings from performance -

of HNP-4850 and 4851.

6. Notify Security Force as to the location of the Emergency Operations Facility (E.O.F.).
7. If Shift Foreman has not reported in to the control room,

- initiate his actions as per HNP-4E23.

A. CONDITION . .

Events are'.in process or have occurred which involve actual or likely major failures of plant functions needed for protection of the public. Any releases are not expected to exceed PAG exposure levels, except near the site boundary. .

B. BEFERFNCE . ,

s INP-4620 C. ACTION . .

1. Proceed to control room immediately.
2. Ensure that correct operator action has been taken to bring emergency under control.
3. Evaluate conditions and findings of the control room operator to see if a General Emergency should be declared, based on performance of HNP-4850, 4851 and criteria in HNP-4720.
4. Assure call list for Site Area Emergency has been initiated.
5. Activate Internal Survey Team by contacting Operations Support Center when staffed. Direct Internal Survey Teams to areas to be surveyed as appropriate.
6. Activate Rescue Team as necessary by contacting Operations l Support Center Managar when staffed.
7. Establish communications with the Security Force, Technical l

Support Center and the Emergency Director as soon as possible and discuss declaring a General Emergency and/or the need for outside assistance for the Site Area Emergency.

B. Direct on site actions to bring emergency under control. .

HNP-4620. t C. A__CTION t@_TE This action is to be taken by the Health Physics Supervisor or designated alternate, and the technicians, whenever possible, or by other trained personnel.

1. Obtain a survey team kit at the Emergency Operations Facility.
2. Turn on survey instruments, allow them to warm up, and check response with source provided.
3. Install glass fiber filter and char. coal- (or silver reolite, Turn if directed) filter in sample holder of air Turn sampler.off.

on sampler and confirm proper operation.

Acquire transportation and stand by for instructions from 4.

Dose Assessment Manager.

5. Proceed to area specified by Dose Assessment Manager.
6. While in transit, take continuous background readings from survey instruments. .

m. Place charcoal (or silver zeolite, if directed) dnd glass fiber filters in air sample holder. .
b. Start air sampler and record time on Data Package 1 (Data Sheet 1). .
c. Check air flow indication and record flow rate.

d.- Run air sampler 10 minutes, observing flow rate during .

the sample period.

e. Make radiation survey as in step C.8. while running air
  • sampler. -

NOTE If levels are greater than 10 m'/hr (approximately 40,000 CPM) report reading to E.O.F. Discontinue ,

sampling and move on to the next location specified by Dose Assessment Manager. Remove filters, place in f e.nvelope and record total sample time and flow rate.

g. Insert G.M. survey meter probe (HP-210) into sample holder (SH-4A) md record background reading.
h. Place glass fiber filter in holder and obtain reading (CPM). Calculate airborne concentration as per Data Package 1 (Data Sheet 1). ,
1. Place charcoal or silver zeolite cartridge in plastic and put directly against probe (HP-210). Obtain reading (cpm) and calculate airborne concentration per

J. Place sample filters in envelopes and record on envelopes time, location, flow, sample time, CPM, BKG, and air activity. Hold for further analysis at Plant or E.O.F. .

8. Take count / dose rate survey as follows: ,
a. At waist level, hold GM probe HP-210 or GM survey meter with the detector window facing horizontal, read meter and record results in CPM. '

9. Collect soil, vegetation and water sample as follows:
a. Take bag marked soil and small scoop, fill with surface soil from a bare spot, tape closed, and tag with the date, time, and location.
b. Take bag marked vegetation and

vegetation approximately two (2) inches above ground, fill bag, tape closed, and tag with the date, time, and location.

c. Fill poly bottle with surface water, close and tag with the date, time and location.
10. Report all results to E.O.F. and await further instructions.
11. When directed by the Dose Assessment Manager proceed to T.L.D. and air sample cabinet locations. Change out TLD's and filters at the stations, noting time and date of change out. Return samples to E.O.F. ,

A. CONDITION ,

Events aresin process or have occurred which involve actual or likely major failures of plant functions needed for protection of the public. Any releases or not expected to exceed PAG exposure levels, except near the site boundary.

C. ACTION .

1. After the RET assembles in the OSC, the OSC manager will send some RET members to the Rally Point, and designate a Rally Point Leader to take charge of the Rally Point. .-
2. Have personnel gather at the Rally Point. If personnel involved in the incident are present or anyone is dressed in protective clothing segregate them from the others.

3. Check with the Security Force. General Contractors and Georgia Power Construction Department to determine those persons remaining in the plant. If any person (s) is (are) unaccounted for, report this to the Shift Foreman. If the Shift Foreman cannot account for the missing person (s) form a rescue team (s) and initiate HNP-4628 SITE EMERGENCY-RESCUE TEAM.
4. Take survey meters and determine habitability of the rally point and proceed as follows:
a. Dose rate less than O.5 mr/hr or contamination less than 100 CPM above background.

(1) Survey or have RET member survey persons involved in the incident first, those wearing protective

clothing second, and then,the remaining personnel .

. (a) Set up change station.

(b) Supervise self-decontamination of personnel.

(c) Using availabic information estimate each persons exposure and record data.

(d) Take TLD badges off persons involved in the incident. Arrange for reading of the TLD badges. ,

(e) Arrange for urine samples and nasal smears from persons involved in the incident to be ,

taken. Evaluate them as time permits.

(f) Do not release any person who has received greater than 25 rem whole body dose or is suspected to have ingested significant amounts of radioactive materials. Such

. persons are to be taken to the hospital for examination and further evaluation of exposure.

(g) Confer with the Emergency Director before I releasing any personnel involved in the incident.

b. Dose rate greater than or equal to 0.5 mr/hr or contamination greater than or equal to 100 CPM above background. .

(1) Proceed with everyone to an area outside of the protected area where the dose rate less than or equal to 0.5 mr/hr or contamination less than or equal to 100 CPM.

(2) Conduct or assist RET members in conducting individual surveys.

(3) Any person that is contaminated (greater than or equal to 100 CPM above background) have him remain for decontamination and notify the Health Physics Supervisor. Assist the H.P. Supervisor, when time permits, in performing step C.4.a(3).

3. Survey Man carries GM survey meter and high range meter. Team Captain carries walkie-talhie and remains in contact with Emergency Operations Facility and/or Shift Foreman.
4. Determine probable location of victim from OSC Manager. l
5. Proceed with rescue. .
a. Stretcher bearers carry: ,

s Stretcher Rope and tackle, if required for rescue.

Pinch bar, if required for rescue.

- First aid kit .

b. Start using respirator while approaching building.
c. Retreat if dosimeter reaches 2R and victim has not been located.
d. Limit dose to members of Rescue Team to 25 rem whole body in effecting rescue.
e. Perform necessary first aid measures for victim but move rapidly when dose rates are high. Move victim into Medical Room and decontaminate if possible.
f. Inform Control Room of condition of victim. Assure

' that Shift Foreman has notified hospital that the victim is or is not contaminated.

g. Contact Emergency Operations Facility or Control Room by radio or phone for ambulance pickup of victim in the upwind direction of victim's location.
h. Move victim to ambulance.
i. Arrange for radiation technician to accompany victim to hospital in ambulance.

1. Report to the Operations Support Center, carrying as many survey instruments, friskers, and air samplers as you can.

NOTE It is extremely important that you move as expeditiously as possible.

2. The Operations Support Center Manager will choose a Rally Point Leader.

. 3. The remaining members of the Radiological Emergency Team will, by assignment of the Operations Support Center Manager: l

a. Assist the Rally Point Leader in performing Rally Point or Personnel Surveys.
b. Serve as Rescue Team Leaders. .
c. Serve as Internal or External Survey Team Leaders.
d. Standby to give other assistance as directed.
4. Members of Radiological Emergency Team come from Plant

~1. Proceed to control room. l

2. Take control of all activities in the control room, ensuring that proper coordination of all activities is carried out.

Assume the duties of the Emergency Director until relieved. I 3.

Assure that those actions in HNP-4E.40 which can be performed from the control room are accomplished.

4. Evaluate actions of Shift Foreman and operator to bring l emergency under control.

S. The Shift Supervisor is responsible for notification of Plant Management and NRC as per emergency call list and other offsite agencies in accordance with notification procedure HNP-4861. The actual nofitication process may be
