s Limitorque Corporation Docket No. 99900100/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conductec' on January 5-8, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activ" ties have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A.

Quality Assurance Manual Section V, Manufacturing Process Control, Revision 0, paragraph A.1, states:

" Completed Bills of Material (Exhibit !

& II) along with their applicable Engineering Change Notices, are the controlling documents used to construct Limitorque products."

Contrary to the above, the construction of actuators (Shop Order 3G0185, item A) was not controlled by the Bill of Material and applicable Engineering Change Notices in that the actuators were built a d shipped with a different unit ratio, output RPM, stem diameter, stem pitch and lead, stem nut, motor pinion, and worm shaft gear than t!;at identified on the Bill of Material and applicable Engineering Change Notices.

B.

Quality Assurance Manual Section VII, Inspection, Revision 1, paragraph A.6.(1), states in part, "The following assembly inspection records are maintained, as applicable:

(1) Electrical Inspection Check-Off (Exhibit XIX)

(2) Mechanical Inspection Check-Off (Exhibit XVIII)"

Contrary to the above, assembly inspection records were not properly maintained in that both Inspection Check-Off sheets for 3G0185 item A serial nos. 17, 20, 21, 22, 23 and 24 indicated a " hammer blow" feature had been verified, although the completed units did not have such a feature.

C.

Quality Assurance Manual Section I, Control of Quality Assurance, Revision 3, paragraph A.1, states in part, "It is the intent of this manual,... to meet the applicable provisions of 10 CFR Part 50 Appendix B...."

Contrary to the above, it was noted that there was no Quality Assurance Manual requirement that procedures, drawings, etc., include accept / reject criteria.

For example Bench Testing procedures did not identify the accept / reject criteria for actuator output RPM tests.

D.

Quality Assurance Manual Section II, Product Design, Standards and Engineering Document Control; Revision 0, paragraph E.4, states in part, "The specific production order Bill of Material will contain instructions on special tests, procedures,... peculi'ar to a particular purchase order requirement."

Contrary to the above, the Bill of Material on Job 3G0185 did not contain a special inspection peculiar to that customer, namely, the use of a customer supplied checklist (BV-110).

E.

Quality Assurance Manual Section X, Nonconforming Material, Revision 1, paragraph A.3.b.(4), requires the followiag of QC inspectors, "If the rework is acceptable, this will be noted on the applicable Variation Report and the work will be released."

59595, 59332 and 59248 did not note that the rework was acceptable.
