LACBWR EMERGENCY PLAN PROCEDURE PROMPT NOTIFICATION SYSTEM 1.0 PURPOSE This procedure describes the prompt notification system installed at LACBWR to comply with Appendix E of 10CFR50.

The system includes a siren and tone alert radios and is under the jurisdiction of the Vernon Coiunty Wisconsin Sheriff as required by federal regulation.

3.0 DEFINITIONS 3.1 The Prompt Notification System (PNS) is required for offsite notification in the event of an incident at LACBWR which may require protective actions as defined by the Environmental Protection Agency's Protective Action Guidelines outlined in the LACBWR Emergency Plan (Reference 2.2).

It consists of the following components:

3.1.1 A Federal Signal Corporation Model 1003B Thunderbolt siren with a Model ARCH *S2 tone activated starter.

This siren generates a 127-l decibal noise level at 100 feet.

It is remotely activated by the Vernon County Sheriff.

3.1.2 Tone alert radios tuned on These radios will be placed by the Sheriff of Vernon County, Wisconsin, and Houston County, Minnesota, in houses in the range of 3-5 miles of LACBWR.

The radios are Model 8020's manufactured by TFT, Inc.

Tney are crystal controlled so they cannot be switched of f the statien and the volume range has a minimum setting.

Page 2 EPP-21 Issue 0 (sJ 3.1.3 Encoder activated radios provided by the Federal Signal Corporation are placed in the homes of certain individuals who assist the sheriff in prompt notification.

4.0 RESPONSIBILITIES 4.1 The Emergency Plan Coordinator is responsible for the maintenance, recordkeeping, and testing of the equipment covered by this procedure.

5.0 ACTIVATION, TESTING, AND MAINTENANCE OF THE EQUIPMENT 5.1 Siren 5.1.1 Use During Actual Emergency (1)

The siren represents a communication mechanism reaching essentially 100% of the population out to three miles and many of the remaining population out to five miles.

The siren is normally activated by the Sheriff

of Vernon County using an encoder on his console radio.

The Sheriff can also cancel s-the siren operation remotely with a special signal.

(3)

If the siren fails to activate remotely, the LACBWR Shift Supervisor may be requested to send someone to manually activate the siren.

Attachment A to this procedure is a 1/29/82 memo from Joe Joseph outlining the steps taken to activate the siren.

Remember to provide hearing protection and the key to the siren Jocks (as an alternate to having to remove the cabinet screws).

The key is No. 2A62748 by Corbin and is maintained in the Shift Supervisor's office.

Siren activate decision and followup procedures are covered by internal direction in the Sheriff's office.

The NRC has determined that control of this equipment must be given to the Sheriff, not the Licensee.

Page 3 EPP-21 Issue 0 5.1.2 Test Situations (1)

The siren will be tested silently once each month.

The Emergency Plan Coordinator will schedule this to coincide with a routine weekly HP visit to this site.

This test shall be performed in accordance with Attachment B to this procedure and signed test sheet given to the Emergency Plan Coordinator for retention.

Any problems encountered shall be referred to the LACBWR Shift Supervisor who shall ensure their prompt repair.

The siren will be given a growl test once every three months.

The test will be scheduled to coincide with (but not replace) a silent test as outlined in Section 5.1.2, Step (1).

.The test shall be performed in accordance with Attachment C to this procedure.

Any problems encountered shall be referred to the LACBWR Shift Supervisor who shall ensure their prompt repair.

The siren will be given a full range test

manually.

The test will be scheduled to coincide with (but not replace) a silent test and a growl test as outlined in Sections 5.1.2, Steps (1) and (2).

The test shall be performed in accordance with Attachment D to this procedure.

Any problems encountered shall be referred to the LACBWR Shift Supervisor who shall ensure this prompt repair.

5.1.3 Maintenance The maintenance of the siren will be conducted in accordance with Section 2.3.

The Section 5-3.A, " Preventive Maintenance -

l Lubricant," will be performed anually in conjunction with Section 5-3.B,

" Annual Inspection and Maintenance."

l Any indication of the need for augmented maintenance of a different type that that outlined j

above should be covered by a change to this procedure.

5.2 Alert Radios 5.2.1 AM alert radios (50 in number) have been provided to the Sheriffs of Vernon County, Wisconsin (40),

and Houston County, Minnesota (10).

5.2.2 Use During Actual Emergency The radios will be triggered from an alert to an active status by transmission of an Emergency Broadcast from Station WKTY in La Crosse.

The Sheriff of Vernon County, Wisconsin, is the only office authorized to trigger this response for LACBWR.

A message will be provided by the Sheriff's office to tell people to stay tuned for further instructions or take whatever required protective action is indicated under the Emergency Plan.

The unlisted numbers to reach WKTY if it is not manned are in the Sheriff's office.

5.2.3 Test situations will occur on these radios each time the check of the Emergency Broadcast System is performed or any announcements are given for severe

weather warnings.

Each radio was provided with

5.2.4 Maintenance is not required on these radios.

Each radio was tested upon receipt from the factory and will perform its intended function without maintenance.

DPC has a quantity of spare radios (10) which can be used to replace a field unit, which fails.

All maintenance will be performed by qualified vendors selected by DPC.

5.3 Decoder Activated Radios 5.3.1 Use During Emergency The decoder activated radios are placed by the Sheriff of Vernon County and clert selected individuals within the EPZ to assist the Sheriff in carrying out protective actions.

5.3.2 Test situations will occur upcn direction of the Sheriff of Vernon County.

5.3.3 Maintenance is not required on those radios.

5.4 Annual Summary Report An annual summary of tests performed on the siren will be provided to the Vernon County Sheriff by the Emergency Plan Coordinator during the month of January.

Open right-hand cabinet by removing two screws at the bottom.

Chopper switch to be switched back and forth between automatic and test at high and low points of siren until desired length of siren use is completed.

V ATTACHMENT B MONTHLY SILENT SIREN TEST (1)

Notify the Vernon County Sheriff's office that a silent test will be performed and the approximate time.

(2)

Dispatch the person performing the test to the siren with:

(a) hearing protec'. ion, (b) a key to enter tae equipment cabinet from the Duty Shift Supervisor, and (c) a portable radio on the Sherif f's f requency.

(3)

Notify the Duty Shif t Supervisor that the test will be performed.

(4)

Open the righthand control box, with hearing protection ready.

(5)

Set the CONTROL switch to test.

(6)

Request the Sherif f's of fice to start the siren.

(7)

Set the CONTROL switch to TEST.

Initiate a siren signal from the siren control device.

Only the Time Delay Relay K102 and the Chopper Motor Starter, K103, should energize.

If a Model AR or AF Timer is installed with the siren, the chopper motor starter should be energized for about four seconds and de-energized for about eight seconds.

This is a silent test of the control circuits; the siren should not produce any sound.

After this test is complete set the CONTROL switch to AUTOMATIC.

WARNING:

The output sound level of a Model 1000 or 1003 Thunderbolt is capable of causing permanent hearing damage, especially at short distances.

Therefore, ALWAYS wear hearing protection when performing tests or maintenance of the siren.

Have the sherif f send the " CANCEL" signal before returning to AUTOMATIC.

(9)

Relock the cabinet and return the key to the Duty Shif t Supervisor.

v ATTACHMENT C OUARTERLY GROWL TEST OF THE SIREN (1)

Notify the Duty Shift Supervisor and obtain the key from him.

(2)

Proceed to the siren with the following equipment:

(a) key to padlock from Duty Shift Supervisor and (b) hearing protection.

(3)

Open the righthand cabinet.

(4)

Set the BLOWER switch to the TEST position.

Observe the blower shaft from the pulley end.

The shaft should be rotating in a counterclockwise direction.

If the direction of rotation is incorrect, turn of f the power to the siren and reverse two of the blower motor power lead connections (3 phase motors, only).

Energize the blower motor again and observe the blower shaft for proper operation.

After completing this test, return the BLOWER switch to the AUTOMATIC position.

Set the ROTATOR switch to the TEST position and ensure that the

(_3) horn rotates.

The direction of rotation is unimportant.

(6)

Set the CHOPPER switch to the TEST position while the horn is rotating.

However, do not turn on the blower.

After-the chopper has reached top speed, return the CHOPPER switch to AUTOMATIC and allow the chopper to coast to a stop.

Very low level sound should be generated.

Repeat this test several times and return both the ROTATOR and CHOPPER switches to AUTOMATIC.

Relock the control cabinet.

(8)

Return the key to the Duty Shif t Supervisor and report any problem with the siren test for immediate repair.

The-annual test is a full operational test of the siren and should be conducted in conjunction with a drill if a drill is required that time.

LACBWR EMERGENCY PLAN PROCEDURE ORGANIZATION AND OPERATIONS DURING EMERGENCIES 1.0 PURPOSE This procedure delineates authority, responsibility and limits on actions of Dairyland Power Cooperative management during emergencies at the La Crosse (BWR).

This procedure defines the actions to be taken under the La Crosse (BWR) Emergency Plan to direct emergency response and guides the Emergency Control Director and his emergency staff in controlling the emergency.

It further delineates the interactions between plant and cooperative personnel, and between cooperative and off-site emergency organizations.

The procedure provides means of documenting all key emergency operations parameters and radiological assessment communications between the Control Room, the Technical Support Center, the Emergency Operations Facility and Offsite Emergency Agencies.

3.0 DEFINITIONS / RESPONSIBILITIES 3.1 The Emergencf Control Director (ECD) is the Cooperative management person responsible for overall direction of the response to the emergency condition.

Initially, this role will be performed by the Duty Shift Supervisor.

If the accident escalates to a Site Area Emergency, or the Emergency Operations Facility is activated, a key Cooperative Manager as listed in Appendix D will become the Emergency Control Director.

The ECD will assign responsibilities to other staff personnel as required.

HE WILL NOT, HOWEVER, ASSIGN THE ULTIMATE DECISION OF RECOMMENDED SHELTERING OR EVACUATION TO STATE AUTHORITIES, NOR INITI ATION OF REENTRY / RECOVERY e-OPERATIONS. THE ECD is the focal point of all emergency (s) activities. The operation and safe shutdown of the facility remains the responsibility of the Duty Shift Supervisor.

The ECD has the authority to request outside assistance from State, Federal, local governmental agencies, DPC staff personnel, consultants or any other response organizations.

He has the responsibility to augment plant staff as requested by the Duty Shift Supervisor, Technical Support Center (TSC) staff as requested by the Technical Support Director or the Emergency Operations Facility (EOF) Staff.

The ECD is responsible for the dissemination of emergency information to all off-site emergency support agencies, and to the public via the joint public information center (JPIC).

The ECD is the single spokesperson for Dairyland Power Cooperative if an emergency condition exists at LACBWR and all information to be released to non-DPC emergency personnel must be cleared through the ECD.

The ECD's Secretary / Recorder maintains documentation of messages received or transmitted to and from the EOF, other than from LACBWR, on the color-coded pressure sensitive Emergency Operations Centers Communication Form included as Table 3.

3.2 The Technical Support Director (TSD) is responsible for the overall coordination of Technical Support Center Activities.

He directs technical assistance given the operations and radiation protection staffs by the TSC staff for implementation of in-plant recovery effort in a manner which minimizes effect to the public health and

safety.

The TSD also provides technical advice to the ECD I

concerning the overall emergency direction upon request from the ECD.

The TSD is responsible for maintaining a log of overall TSC activities.

3.3 The Plant Operations Parameters Director (OPD) is responsible for obtaining current reactor operations parameters by direct communications from the control room for use by the TSC staff.

The OPD advises the Duty Shift Supervisor in control of specific plant parameters necessary to prevent accident escalation.

Upon request from the ECD in the EOF, THE PLANT OPD may provide technical advice to the EOF.

It is preferable that the OPD have a Senior Reactor Operators License.

The OPD answers directly to the Technical Support Director.

He maintains documentation of reactor operations parameters by using the color-coded pressure sensitive Reactor Operations Parameters Communications Form in Table 4.

3.4 The Plant Radiological Assessment Director (RAD) is responsible for obtaining current plant radiological conditions by direct communications with the Control Room, HP Technicians surveying inside the plant and HP Technicians surveying within the site boundary.

The RAD directs inplant and onsite HP Technician survey teams and assists the OPD in evaluation of plant parameters which might result in unnecessary release of radioactive materials to the environment.

The RAD is responsible for initial calculations of off-site doses or projections of those doses based on effluent releases of radioactive material and current site-meteorology. Upon request from the ECD in the EOF, the RAD may provide technical advice to the EOF staff.

The RAD answers directly to the Technical Support Director.

He maintains documentation of plant radiological parameters by using the color-coded pressure sensitive In-Plant Radiological Parameters Communications Form in Table 5.

3.5 The Cooperative Operations Parameters Director (OPD) is responsible for obtaining all current operations parameters from the Control Room.

The OPD reports directly to the ECD upon activation of the EOF.

The OPD is responsible for technically advising the ECD on current reactor conditions and safety system functions.

The OPD establishes direct communications with tht Control Room, and may concurrently

establish direct communications with the TSC, through a Technical Communicator who documents all messages on Table 4.

Operations advice from the ECD to the plant are communicated through the OPD.

3.6 The Cooperative Radiological Assessment Director (RAD) is responsible for obtaining all current radiological parameters available to the TSC.

He reports directly to the ECD upon activation of the EOF.

The RAD is responsible for technically advising the ECD on current plant radiological conditions and advises the ECD on off-site radiological conditions.

'The RAD works closely with the ECD and recommends public protective actions to th-ECD.

He is responsible for final calculation of off-site dase estimates based on radioactive releases and meteorologic.n conditions.

He is responsible for the overall direction c. off-site radiological assessment including the In-Pield RAP teams. The RAD establishes direct communication with a HP Technian in the Control Room, and/or the plant RAD through a Technical Communicator who documents messages on

3.7 The Cooperative In-Pield Radiological Assessment Director (IRAD) is responsible for all actual off-site radiological assessments.

In conjunction with the plant RAD, the IRAD coordinates the activities of all in-field radiological assessment parameters (RAP) teams.

The IRAD reports to the cooperative RAD.

The IRAD is responsible for comparing calculated off-site dose estimates with actual RAP team assessment.

The IRAD secretary / recorder documents all messages on Table 5.

He works closely with the Cooperative RAD at the EOF to assess necessary Public Protective Actions.

3.8 The Off-site Dose Calculations and Trajectory Specialist is responsible for actual calculations of off-site dose estimates based on available meteorolgocial data and radiological release data supplied by the HP Technician in the Control Room or the plant RAD or actual field data supplied by the RAP team.

He also determines projected trajectory of the radio-active material released from the plant and plots this on maps in the EOF.

His computed data is supplied to the RAD and IRAD for their assessment of proper in-field RAP Team placement and analysis of recommended public protective actions.

3.9 The Cooperative Emergency Munpower and Materials Procurement Director (EMMPD) is responsible for obtaining additional cooperative or outside manpower or equipment as necessary for LACBWR re-entry / recovery operations.

He reports directly to the Emergency Control Director.

He reports to the La Crosse Office upon notification by the ECD.

(Refel to Appendix D).

3.10 The Public Information Director (PID) is responsible for the activation and operation of the Joint Public Information Center (JPIC).

He reports to the Emergency Control Director and obtains all information to be disseminated to the public from the ECD.

He will assure that this concept of " single spokesman" is in fact carried out to avoid even minimal contradictions of detail or analysis.

He is responsible for arranging interviews, for statements quoted in press releases or other announcements, and for presiding at formal press conferences.

He coordinates information at the news center with his counterparts from local, state, and federal agencies and with other companies involved with the emergency, and provides a means of meeting the media's needs.

3.11 The Cooperative Emergency Communications Director will be used as necessary by the ECD to augment or repair communications equipment.

4.5 RECOVERY AND RE-ENTRY 4.5.1 After the initial preplanned emergency actions have alleviated the emergency situation, additional actions may be necessary to effect the most complete recovery possible.

All recovery actions will be preplanned and all actions will be documented.

Affected areas will be roped of f and posted with warning signs indicating radiation levels or other hazards.

Every reasonable effort should be made to limit radiation exposure.

However, the exposure levels justifiable will depend on the specific circumstances, taking into account risk versus benefit factors in the particular situation.

4.5.2 Section 4.6 indicates resources which can be utilized during the recovery and re-entry phases of an emergency a t LACBWR.

The Emergency Control Director will have the authority and responsibility to manage the overall re-entry and recovery operation.

He has the authority to request of fsite support and assistance from other utilities, consultants, INPO, etc.

4.5.3 The initial return to the plant after a site area evacuation will be made by a re-entry team composed of at least two members.

The team will conaist of at least O'

one member of the LACBWR Health and Safety Department and one member of the LACBWR Operations Department.

More than two persons may be used in the party if it can result in the saving of human life or reduction of injury.

The decision to re-enter shall be made by the Emergency Control Director and the following prerequisites should be considered:

(1) protective clothing, l

(2) protective breathing apparatus, (3) personnel monitoring TLD badges and dosimeters, (4) radiation monitoring instrumentation, (5) communication with the Emergency Control Team or with the Control Room, preferably both, and (6) if available, the conditions inside the plant should be determined from the in-plant radiation I

result, (2) 25 Rems - if the reduction of human injury will result, and (3) in all other cases, the dose should be limited to 12 Rems or less.

4.5.5 Re-entry after an area evacuation will be by decision of the LACBWR Health and Safety representative on duty at the time.

No one should re-enter an area until approval is given by a qualified person.

Special monitoring equipment, in addition to that in the emergency boxes, is kept in the Waste Treatment Building and in the Change Room to assist in evaluation of re-entries.

The in-plant monitoring equipment should also be used when possible.

4.5.6 Once the potential of escalating an event has passed, O,.

steps should be started to recover from the incident.

Guidelines and responsibilities for recovery are outlined below:

(1)

There must be assurance that the problem which caused the emergency is solved and that this same incident cannot immediately reoccur.

(2)

The general occupancy areas must be free of significant contamination and radiation and high radiation areas properly defined and posted.

(3)

Airborne radioactivity must be eliminated or controlled within 10CFR20 limits.

(4)

Access to the area must be controlled and exposures of personnel documented.

Exposure for recovery should be limited to 10CFR20 limits.

l (5)

The LACBWR Operations Review Committee under the Emergency Control Director is responsible for evaluating recovery advisability.

(6)

Plant records and other essential information necessary for evaluating the emergency must be available.

All major recovery steps will be performed in accordance with written procedures.

These procedures will be prepared under the supervision of the Technical Support Director, by qualified LACBWR staff members, or outside consultants.

They will be reviewed by the Operations Review Committee and approved by the Emergency Control Director.

4.5.7 When the recovery operation has been completed and the plant has been returned to an operable condition, the Emergency Control Director is responsible for authorizing the return to normal operation.

If the emergency shutdown resulted from a safety limit being exceeded, reactor operation will not be resumed until authorized by the NRC.

A complete analysis of the situation will be made, together with recommendations for preventing a reoccurrence.

This report shall be submitted to the DPC General Manager, the Safety Review Committee, and the NRC.

The plant will be restarted only when:

(1) the conditions causing the emergency are returned to normal, (2) the cause of the emergency is understood, (3) res to ration, repair, and testing are complete as

required, (4) no unreviewed safety questions exist, (5) all conditions of the License and Technical Specifications are satisfied, and (6) all required approvals have been received.

4.6 RESOURCES 4.6.1 Resources are available to supplement staff in emergency situations.

Backup capabilities from DPC staf f and outside entities are outlined in this section.

EPP-3, Section 5.4, lists numbers and messages for several of these contacts.

4.6.2 Upon initiation of any emergency response, specific plant-groups, departments, or individuals have general responsiblity to take actions.

These assignments are described in Table 4.2.

4.6.3 The offsite non-DPC support agencies having specific expertise in various areas are listed in this section.

4.6.3.1 Sheriff's Office - Vernon County The Vernon County Sheriff's Department Headquarters is located in Viroqua, Wisconsin, twenty (20) miles from the plant site.

The department will cooperate in any emergency situation brought to its attention.

Department personnel are available to perform such duties as traffic control, personnel evacuation, site security, communications, etc.

4.6.3.2 Tri-State Ambulance Tri-State Ambulance Service is located in La Crosse, Wisconsin.

Written agreement exists with them whereby ambulance service and in-route life support treatment will be provided during transit in the case of an emergency at the plant.

S. Coast Guard.

The Lockmasters can provide radio communications, as well as control of the locks.

Other immediate local assistance available from Lock and Dam No. 8 is the use of a small craft, emergency shelter, medical first aid, and telephone communications.

4.6.4 Additional specific physical resources both DPC and non-DPC owned are listed in this section.

4.6.4.1 A complete Environmental Survey Program (ESP) for La Crosse Boiling Water Reactor has been l

established by DPC and is presently in operation.

The plant Environmental Survey Program 2

includes 21 area TLD's at specified locations i

in the vicinity of the plant.

These devices are normally processed of fsite and could be i

read on an emergency basis by the contracted supplier.

At least five air monitoring stations are in operation as part of the Environmental Survey Program.

Each contains a particulate filter and charcoal cartridge which could be analyzed with portable survey equipment.

These portable air samplers are i

located at dif ferent points around LACBWR and are capable of measuring radioiodine concentrations of 10_7 pCi/cc and lower.

LACBWR is developing the capability to place a portable air sampler at a spot within the plume which can then have its media removed to the laboratory for isotopic analysis.

Additional government facilities downstream and in La Crosse area are available for contact to obtain additional information on these parameters.

Meteorological data is available from the U. S.

Weather Bureau Station at La Crosse, Wisconsin, approximately 20 miles north of the plant.

Meteorological data is available from the Alma, Wisconsin Energy Center of Dairyland Power Cooperative.

Meteorological data is also available at the Madison, Wisconsin, U.

S. Weather Bureau.

4.6.4.3 Offsite Laboratory Facilities The Univeristy of Wisconsin-La Crosse Nuclear Radiation Center, located in Cowley Hall will serve as an offsite laboratory.

Also, avail-able will be the Dairyland Power Cooperative laboratory located in the Service Center Building on East Avenue in La Crosse.

The University of Wisconsin-La Crosse labora-tory has the capability to analyze environmental samples for gross alpha, gross beta, gross gamma, as well as isotopic identification.

The capability also exists for the analysis of iodine in milk.

Analysis can be provided on a 24-hour immediate notification basis.

Issue 3 The University of Wisconsin-La Crosse Radiation Center has two multi-channel analyzers with Sodium Iodide and Germanium detectors, liquid scintillation counters, thin window proportional counters, alpha spectro-meter, Geiger-Mueller counters, and single channel analyzers to determine the radioactivity content of multiple environmental samples.

4.6.4.4 Protective Facilities and Equipment The OSC at G-1 is able to accommodate all personnel present onsite at any given time.

There are approximately 70 workers at LACBWR and 80 at Genoa 3 present during the day shif t.

The OSC is equipped with a telephone.

It also has a first aid kit.

Also, located at the OSC is an emergency equipment kit,.which contains protective equipment, radiological monitoring equipment, and emergency supplies.

4.6.4.5 Damage Control Equipment Numerous DPC vehicles and a variety of heavy equipment are available onsite for use.

This equipment is maintained by the adjoining coal plant (Genoa 3 unit) and the Alma Dock Corporation.

Equipment includes bulldozers, mobile cranes, tugboats, forklifts, winches, welding equipment, chain falls, etc.

All DPC heavy equipment normally present at the Cooperative's Office is available at the site within two hours.
