This refers to the routine safety inspection conducted by Mr. S. L. Lasuk of this office on February 16, 17 and 19, 1982, of activities at Wright State University, Fels Research Institute, and the Cox Heart Institute authorized by NRC Byproduct Material Licenses No. 34-11912-03, No. 34-11912-04, No. 34-11912-05, and Special Nuclear Material License No. SNM-1419 and to the discussion of our findings with you, Mr. D. Atwater, Dr. A. Rake, and Dr. B. Austin at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
No items of noncompliance with NRC requirements were identified during the course of this inspection of Licenses No. 34-11912-04, No. 34-11912-05, and No. SNM-1419.
Ilowever, during the inspection of License No. 34-11912-03, certain of your activities appeared to be in noncompliance with NRC requirements, as specified in the enclosed Appendix. A written response, submitted under oath or affirmation, is required.
The responses directed by this letter (and the accompanying Notice) are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
