APPENDIX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-286 Indian Point Unit 3 License No. OPR-64 1
i 15,1982, and As a result of the inspection conducted on January 16-February (October 7,1980),
in accordance with the Interim Enforcement Policy 45 FR 66754

the following violation was identified:
10 CFR 50.59 pemits the licensee to make changes to the facility as described in the Safety Analysis Report without prior Commission appmval, pmvided the licensee maintains records of the changes. These records shall include a written safety evaluation which provides the basis for the detemination that l
the change does not involve an unreviewed safety question.
l contrary to the above, on February 9,1982, changes to the facility - as des-cribed in the Safety Analysis Report, were identified for which no written l
safety evaluation was prepared.. These changes were the addition of a valve in line with the nitrogen regulator which supplies the isolation valve seal water system (IVSWS), a technical specification requimd system, and the addition of a 3/8 inch line which connects the IVSWS to the main nitmgen header.

Pursuant to the provisions of 10 CFR 2,201, Power Authority of the State of New York, is hereby required to submit to this office within thirty days of the date of the Notice, a written statement or explanation in reply, inclu-ding (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will _be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of l
Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown, consideration will be given to extending your msponse time.
The responses directed by this Hotice am not subject to the clearance pro-cedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1900, PL %-511.
