Interim Report of 10CFR50.55(e) Item on Failure of Core Spray System Bellows (#53)
Dear Mr. Keppler:
On February 2,19 82, Detroit Edison's Mr. II. A. Walker, Supervisor-Construc-tion Quality Assurance, telephoned Mr. J.E. Konklin of NRC-Region III, to report on a problem with failure of Core Spray Pump System Bellows at the Fermi 2 site.
During a filling operation, prior to flush, the core spray system was pressur-ized to approximately 300 pounds. This resulted in a failure of the expansion bellows. The design pressure for the bellows is listed on quality records as 125 pounds. Ilydrostatic test pressure for the bellows was 1871/2 pounds. A Nonconformance Report (NCR) was written on the bellows.
Engineering had reviewed and approved the use of the higher pressure for the flushing operation. Correspondence from Engineering granting the approval indicates that maximum working pressures for core spray piping is in excess of the 300 pounds. There is no evidence that the bellows were evaluated.
Engineering has performed a complete re-evaluation of the Core Spray System.
One flange in the system was noted that should not be sabjected to the 300 psi flushing pressure. A NCR has been written on the flange. Flushing of the system was resumed at 275 psi maximum pressure which was the reduced pressure specified by Engineering. Other systems and components have been evaluated to ensure that components have not been subjected to excessive flushing pressures.
Another report on this item, either interim or final, is scheduled to be sent to you on or before July 1,1982.
