APPENDIX A NOTICE OF VIOLATION Haine Yankee Atomic Power Cor..pany Docket No. 50-309 Maine Yankee Nu: lear Power Station License:'io. DRP-36 As a result of the inspection conducted on December 21, 1981 - February 8, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was iderrtified:
Technical Specification 5.4.1 requires the iraplementation of a Licensed Operator Requalification program consistent with Appendix A to 10 CFR
- 55. Appendix A to 10 CFR 55 and licensee procedures 0-00-5, Revision 8, and 18-200 ',, Revision 2 specify the use of both periodic examinations and annual examinations as means for evaluating program effectiveness and operator knowledge.
Contrary to the above, during the 1980-1981 requalification program cycle and thus far in the 1981-1982 cycle, no formal periodic examinations other than the annual examinations were administered.
This is a Severity Level V Violation (Supplement 1.E).
Pursuant to the provisions of 10 CFR 2.201, Maine Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the correc-tive steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; (3) the date when full compli-ance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirma-tion. Where good cause is shown, consideration will be given to extending your response time.
