Dear Mr. Keppler:
On February 1, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as -
required by 10 CFR 50.55(e) . While performing construction surveillance, PSI quality assurance personnel observed incorrect welds on safety related llVAC supports supplied by Pullman Construction Industries, Inc. (PCI). Further investigation determined that PCI personnel were incorrectly interpreting a weld symbol during shop and field fabrication of these supports. Had this situation remained uncorrected these supports may not have been able to withstand their most severe design basis loads.
To prevent reoccurrence of this situation PCI has implemented a training program to educate its personnel on the correct interpretation of weld symbola used in their scope of work. PCI will also perform rework on the affected hangers so they will conform to the design drawings.
