a Appendix A NOTICE OF VIOLATION Arizona Public Service Company Docket No. 50-528 P. O. Box 21666 Construction Permit No.141 Phoenix, Arizona 85036 As a result of the inspection conducted on January 25-29,1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),

the following violation was identified:

Appendix B of 10 CFR 50, Criterion V, as addressed in paragraph 5.1 of the Quality Assurance Program in Appendix A to the PSAR, states in part:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions procedures, or drawings...."

Bechtel drawing No.13-C-ZCS-655 Revision 2, which governs structural steel column fabrication, in Section G, " weld detail," depicts 1/2" fillet welds on the outside of the column flange to base plate intersection.

Contrary to the above, on January 28, 1982, three columns at the 149' elevation of the Unit I containment building were found to lack the required welds. The column welds had been inspected and accepted at a Bechtel approved vendor fabrication shop prior to shipment to the site.

This is a Severity Level V Violation (Supplement II), applicable to Unit 1.

Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid i

further items of noncompliance; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response I

time for good cause shown.
