SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2.- NRC/0IE REGION II INSPECTION REPORT 50-327/81-41 AND 50-328/81 RESPONSE TO VIOLATION The subject OIE inspection report dated December 31, 1981 cited TVA with one Severity Level IV Violation. Enclosed is our response to the inspection report. In a telephone conversation on January 28, 1982, D. Quick, NRC-0IE, agreed to an extension until February 5,1981 to submit our response.

ENCLOSURE SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 RESPONSE TO VIOLATION Violation 50-327/81-41-01 and 50-328/81-50-01

- 10 CFR 20.103(a)(3).. requires that the licensee use suitable measurements of concentrations of radioactive materials in air for detecting and evaluating airborne radioactivity in restricted areas.

Contrary to the above, on July 12, 1981, the licensee failed to make suitable measurements of radioactive material in air for detecting and evaluating airborne radioactivity in that air samples were not taken in the Unit 1 accumulator number two room immediately before or during work in the room to determine the requi'rements for respiratory protection. Iodine 131 concentrations of 1.58 maximum permissible concentration were measured after nine workers exited the area and were found to be contaminated.

This is a Severity Level IV Violation.

Admission or Denial of Alleged Violation TVA admits the violation occurred as stated.

Reasons for Violation As stated in the report, health physics personnel previous to the incident obtained general work area radioactive airborne samples in three areas of containment and had not identified any significant concentrations of Iodine 131. For several months before the incident, general and specific radioactive airborne surveys were conducted in unit 1 containment. The data collected from th3se surveys had not identified any significant concentrations of Iodine 131.

As a result of previous surveys, history, and the nature of the task (no welding, grinding, or breaching of a system with unknown radioactivity levels) to be performed during the incident in question, the decision was made to allow the workers to enter the area without additional radioactive airborne surveys or respiratory protection devices.

Corrective Action Taken and Results Achieved A program is currently in effect to supplement the general work area radioactive airborne survey program with additional surveys where there may exist containment atmospheric stratification or dead air space (minimum air movement).'

Steps Taken to Avoid Further Recurrence As stated above, additional surveys are being collected in specific work locations and/or commensurate with the task to be performed. A program will be implemented based on these additional surveys and data collected.

This program will ensure compliance with 10 CFR 20.103(a)(3).

Date of Full Compliance The actions described above will be completed by April 1, 1982.
