FINAL REPORT - DER 81-47 DEFICIENCY EVALUATION 50.55(e)

ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNCS UNIT 1 I.

Description of Deficiency An existing grout patch in the bottom of the Elevation 100'-0" floor slab of the Main Steam Support Structure was found to be hollow and cracked.

Four (4) exploratory holes were drilled and the patch material was found to be soft and not bonded.

The grout patch was found to extend under an embed ~ plate and under approximately one-half of an embedded unistrut. The 3'-6" concrete slab was placed on April 20, 1979 (Placement

  1. 1M003-U1). This area was apparently repaired with pneumatically placed grout soon after the forms were stripped on April 21, 1979.

PVNGS DER 79-12 covers the past problems associated with pneu-matically placed grout.

Procedures in effect at that time identified and repaired surface voids by use of Rework Tag's, not'NCR's which were required only if a void extended completely through a wall. At a later date, the procedures were changed to require NCR's on voids extending past the first layer of reinforcing bar.

On April 29,1980, NCR C-X-1897 was written to inspect all previously grouted or repaired voids. The above void was missed during that inspection; this oversight occurred because the Rework Tag for the original defect had apparently been lost.

Discovery of this particular defective grout patch is considered an isolated case since the corrective action taken in regard to DER 79-12 is considered effective.

II.

Analysis of Safety Implications It is determined by an analysis of the existing condition (assuming the grout patch area is a void) that there is enough embedment in the sound concrete to develop the anchor bolts pn the embed plate for the required load, and the surrounding concrete is sufficient to develop the reinforcing steel to resist the forces in the slab under all postulated loading. There is a single conduit attached to the embedded unistrut. The attachment is within the area of the grout patch.

It is also determined by analysis that the partial absence of sound concrete under the unistrut will not affect the integrity of this support for this particular load.

III.

Corrective Action Nonconformance Report CC-3089 will be dispositioned to have the grout patch repaired.
