Dear ifr. Cavanaugh:

WJohnson - Region IV We have reviewed your Final Report on Af@-1 Leak Fuel Rod Investigation.

As a result of this review and the information obtained by our visit to AH0-1 on June 25 and 26,1981, we have determined that a me6 ting is warranted with your staff and your fuel vendor, Babcock and Wilcox.

The purpose of the meeting will be to exchange information regarding the examinations that were conducted (or not conducted) and the basis for the conclusions drawn from those examinations. In particular, we are seeking additional information to support your conclusions,that

1) the most probable cause of fuel failure was "some form of internal clad attack," 2) there are no indic4tions the leaking rods r,eskited from PCI, 3) there are no indications the leaking rods resulted from manufacturing related sources and 4) "further work to establish the exact nature of the attack appears unwarranted."

In addition to the above stated areas,We would like more detail on (a) r*diochemistry techniques to infVr the number of factures and (b) methods that could be used to further examine the failed fuel rods.

We would like to have presentations on the above related material.

Please arrange with your Project Manager,for AH0-1 a suitable time for the meeting during the time of the last week in February and the first week in llarch 1982.
