APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-412 Beaver Valley Unit 2 License No. CPPR-105 As a result of the inspection conducted on December 1-31,1981, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7,1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion V and ANSI N45.2(6) require that quantitative criteria, such as dimensions and telerances, shall be specified, as appropriate, for detemining satisfactory work performance and quality compliance.
Contrary to the above, the Control Room Ventilation and Air conditioning System was joined together by boltir.g without specified controls for hole sizes and tolerances in the job specification, work instructions, drawings, and/or stan-dards.
This is a Severity Level IV Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required.
i to submit to this office within thirty days of the date of this Motice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended,'this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
