APPENDIX A NOTICE OF VIOLATION Public Service Company of New Hamsphire Docket Nos. 50-433 & 50-444 Seabrook Units 1 and 2 License Nos. CPPR-135 & CPPR-136 As a result of the inspection conducted November 17, 1981 - January 8, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, j

1980), the following violation was identified:

10 CFR 50, Appendix B, Criterion III states, in part that:

" Design changes shall be subject to design control measures commensurate with those applied to the original design..."

These measures shall assure that regulatory requirements and the design bases "are correctly translated into specifications, drawings, procedures, and instructions."

The Seabrook Station PSAR for Units 1 and 2 states, in part, in paragraph 17.1.3 that" "Each organization performing design activities for items covered by this program shall maintain design control measures.

The measures shall consist of procedures..."

UE&C Specification 248-51 states, in part, in paragraph 3.5.7.1 that:

"All hangers... shall be located with respect to the as-built location of the installed piping... taking into consideration the variation necessary to accomodate thermal growth."

Contrary to the above, as of November 27, 1981, modifications to pipe hangers N/S-1812-SG-08 (Unit 1) and M/S-2-1812-SG-08 (Unit 2) had been accomplished in accordance with a design change (Engineering Change Approval, ECA 25/0665A) which did not take into engineering consideration the thermal growth of the supported piping and which therefore provided the potential, in service, for adverse impact upon branch piping connections, leading to a loss of service water coolant.

This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of New Hamsphire is hereby required to submit to this office within thirty days of l

the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) 1 corrective steps which will be taken to avoid further violations; (3) the date 1

when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
